I am so grateful to the staff at Lake Montgomery in Lake City, FL. for the care, compassion, and kindness they showed my dad during his time there. The nurses, aides, and staff treated him with dignity and truly cared about his comfort and well-being. They were patient with our family, kept me informed, and made a difficult time a little easier with their support and compassion. I will always appreciate the love and respect they showed him, and we are thankful for everything they did for our family to his very last day here on earth (5/25/26). VITAS was his hospice care team and they were just as great! Ms. Connie, Ms. Tiffany, and Greg were just to name a few that were so wonderful to my daddy and I cannot thank them enough. This is truly a great facility with humans that actually care. My dad has been in a few different facilities in the last few years and the only regret I have is that I didn’t find this one first!!!

My experience with this facility was great. They took great care of my dad. His therapy after his stroke was amazing! He didn’t stay long at all. The motivation here was great and all of the staff were amazing and approachable.

Needs more cleaning. My friend has fell out of bed twice. Asked for him to be sent to the hospital. The facility said no. In 4 month's time he has tears all over his body. The head staff is rude. I am my friends Power of Attorney. Why would they not send him out to the hospital? Because it shows he is being neglected.

My sister got the best care at Lake Montgomery Health and Rehab center All the nursing staff was very attentive and caring. Mary is a wonderful person and has a great personality. I really appreciate the help and support of everyone. Thank you for everything 💓💓

My 85yr old dad came here to go through physical therapy after getting sick and going into the hospital. All the Administration, Physical Therapy, Nursing,CMA and all the rest of the staff were wonderful. Everyone treated my dad and I like family. If I had any questions about my dads care or how his physical therapy was going the staff answered all my questions and concerns right away. The Physical therapy staff worked with my dad and I was able to watch him do his exercises. When he first got there he could barely stay awake after 2wks he was up and walking with his walker and with assistance. I am very glad my dad came to Lake Montgomery Health and Rehabilitation Center. Thank you for taking such great care of my dad i really appreciate everything you have done for us.
