Hebron Oaks Skilled Nursing & Rehabilitation Center

Hebron Oaks Skilled Nursing & Rehabilitation Center is located on the University Woods campus in Madison, Wisconsin, and is part of Oakwood Village, a senior care organization sponsored by 37 Lutheran congregations under Oakwood Lutheran Senior Ministries, a not-for-profit organization.

The center offers short-term rehabilitation and ongoing physical therapy for individuals recovering from surgery, illness, or injury, with care available for both brief and longer-term stays. Highly trained occupational, physical, and speech therapists create personalized treatment plans designed to help residents regain strength, restore independence, and return to daily life. Rehab services are offered both on an outpatient basis and through short-term nursing care stays.

Hebron Oaks operates in an all-new building on the University Woods campus, featuring expansive patient suites, an on-site cafe, and dedicated functional rehab spaces designed to support recovery in a comfortable, state-of-the-art environment.

Residents and patients also have access to the Agility Park, a specially designed outdoor space intended to help rehabilitation patients overcome the physical and psychological challenges of transitioning from a hospital or rehab center back to everyday life. The park offers varied terrain and inclines that support gentle movement, exercise, and sensory exploration, helping to improve physical strength and balance while reducing fall risk.

Hebron Oaks Skilled Nursing & Rehabilitation Center was rated five out of five stars, and Oakwood Village's skilled nursing programs received recognition in the 2025 US News & World Report Rankings for Short-Term Rehabilitation.
