My mom stayed there for a month, and the doctors and staff were generally approachable. The facility was clean, and the food was good.<br><br>Over the past couple of years, my mom has also stayed at Parkwood and the Lodge at Bear Creek in Grapevine. Compared to those facilities, this one provided a better overall experience.<br><br>There were some areas that could be improved. Communication among staff was not always consistent. At times, one staff member or CNA would provide different information than another, which could be confusing for patients and families.<br><br>There were also occasions when requests for assistance were not answered promptly. In addition, I observed some staff members using their phones and appearing less attentive to patients. At times, it seemed that concerns were addressed more quickly when a family member was present and specifically advocated for the patient.<br><br>Transportation services were another area that could be improved. Scheduling transportation for medical appointments was not always easy or convenient, and availability appeared to be limited. As a result, I often had to transport my mom to her doctor appointments myself.<br><br>While there were some challenges with communication, responsiveness, and transportation services, the quality of healthcare and food were good. Overall, this facility provided a better experience than Parkwood and Bear Creek, and I appreciate the care my mom received during her stay.

This is a review for the long term care side of this nursing home. The short-term Rehabilitation and Physical Therapy side is not mentioned in this review. My mother lived at this facility for two years. While daytime care was generally adequate, nights and weekends were consistently poor and neglectful. For over a year, I raised concerns specifically about evening and weekend care, yet these issues went unaddressed and showed no meaningful improvement. There were multiple instances—especially on weekends—where my mother was left unchanged for extended periods. She often had to call me so I could contact staff to get basic care addressed. My mother frequently complained that her call light would go unanswered, leaving her unassisted in her room despite being bedbound. On several occasions when I came to help her myself, I observed CNAs at the nurse’s station playing on their phones instead of responding to residents, or no one at the nurses station at all for extended periods of time. The food quality and variety were also concerning, with the same meals repeated several days in a row. Communication was one of the biggest failures. In two years, I was never able to speak directly with her physician despite multiple requests. I was never even given the doctor’s name, and staff could not provide clear answers. This was especially troubling since I needed to be involved in her care. There were repeated hospitalizations that appeared preventable with proper monitoring and timely care. She was sent to the hospital multiple times for infections, including severe UTIs and pneumonia. In one case, an ER physician described her UTI as the worst he had seen in his career. In 2026 alone, she was hospitalized four times in four months. The facility was also reluctant to arrange hospital transport when needed. At times, my mother had to call 911 herself, or I had to intervene and insist on transport. During these critical situations, I was not kept informed of lab or X-ray results. When I raised concerns and asked what steps would be taken to prevent future incidents, I was told that if I was unhappy, I could move her. After that, repeated attempts to contact leadership—including the Director of Nursing and administrator—went unanswered. We ultimately moved her. No one from the facility has followed up to ask why. The building may look nice, but the level of care, communication, and responsiveness was deeply concerning. I cannot recommend this facility.

The staff was so caring and very helpful The front desk to the gym everyone was outstanding. If I needed anything the nurses or anyone that would pass through was very helping. I recommend any and everyone to try forum Parkway rehabilitation facility

I attended a Thanksgiving lunch for families. The staff amazed me at their attentiveness! I saw residence struggling to reach their food and just as quickly, a cna or other staff member was helping them. The activity director LadyBug was full of energy and made a point to interact with the families. The residences seemed to smile and enjoy interacting with every staff member they encountered and gave me a very good feeling about the whole facility!
