This place should be shut down. If you have a loved one there and don’t check on them you should. They line them up in the hallway without interaction. Treat the patients like they are too time consuming. If you are continent you won’t be when you get there when they make you pee and poo in your double diaper that was addressed many times. Imagine how humiliating that is when you can use the toilet! Of course a wound developed but don’t worry they told my family member “you’ll be someone else’s problem soon” we were there for a short rehab. Terrible terrible place Don’t believe the good reviews they are prob from people that work there. I can’t emphasize how bad this place is and I have been to some bad ones. It is filthy and stinks. After the wound was caused by them took another week to get a wound care order, horrible unnecessary pain. As long as you nurses continue to act like you can do the job so understaffed, none of these facilities will change. Take your nursing skills to where you can really make a difference in someone’s life and let these places close. They don’t care about you or any patient there and it is clear when you walk in the door. One nurse to 30 patients is impossible no matter how hard you try. I’ve never seen someone neglected so badly shame on you Forest Manor!

This facility is very poorly managed and under staffed regardless of what they say. They were in no way prepared for my mother’s arrival from the hospital. They didn’t have the proper bed or any of her medication, any didn’t get it until I complained to the director, 5 days.Even after complaining and making many requests my mother was double diapered, food left out and tried to be reused and not gotten out of bed. Though some staff were very kind and helpful it didn’t outweigh the lack of care, the broken g tube, the bed sores and overall neglect. Not a rehab for a person with many medical requirements. Pt staff are amazing but that only allots for a hour a day the rest of the time a small staff is left to care for a very large group of very needy people. Long term care is mixed in with rehab/ short term skilled nursing. I would never recommend this facility.

This facility is kept very clean and tidy. The staff are very attentive and caring, and all speak fluent English. Daily activities, lovely grounds. It is located in a rural out of the way area, very quiet. They do have a dementia unit as well as long term and post-acute. Not the fanciest facility around, but we were very happy when there for rehab.

Very happy with the facility. Friendly staff and clean, was a great experience for my loved one. Easy and accommodating admin, social worker, PT, nurses and staff. Would recommend!

My mother was admitted to this nursing home approximately 5 days ago since Karen Ann hospice was full! Spoke to Mellisa prior to her going there gave me all the ins and outs got her there and spoke to Gabrielle as well plus other staff members as well! Long story short my mom passed god rest her soul and these peeps were wonderful to my mother for her short stay! Thank you gabby Mellisa and your wonderful staff nurses for her short care at your facility.
