Parking is available via the Gold Ramp, located beneath the 2512 Building at the corner of 25th Avenue South and South 7th Street, for both patients and visitors. Valet parking is offered in the Gold Ramp Monday through Friday from 8:30 am to 5 pm, with discounted parking pass options available from the valet attendant during business hours.

Patients and visitors with hearing or speech disabilities may contact the center through their preferred Telecommunication Relay Service.

Mom was here for a short stay doing rehab - a lot better than other facilities we have been at - pt was thorough, staff was attentive. Very professional environment.

Not much about the rehab facilty supports healing. It is unkept in the rooms. Room is dark. Aids are not good

My husband suffered a stroke, and when I brought him in last week, I requested an evaluation of his mood. Today, I returned because I was informed that he would be discharged, but there has been no communication about the next steps. No one seems to know what’s happening, and there has been no assessment of his mood. I am deeply disappointed with the lack of communication from the doctors and nurses. I’m still waiting. On another occasion, I came in early in the morning and stayed all day, yet no doctor ever showed up. However the staff individually were super friendly and nice , a big Kudo to PT and OT team , nurses and assistant nurses… Thank you all …

So many good things to say but also so many not so great things. OT/PT experience: OT/PT teams are amazing! Jeff from PT is the highlight of our stay here. He is an advocate for his patients, especially if he hears and sees something that isn't done. He will go out of his way and job description to help. He makes therapy entertaining and patients laugh. OT staff is so kind and patient with you during therapy. They really encourage and push you to do your best. If I could rate this place on just therapy services alone, I would give them 5/5. Nurses: There are some kind, patient, empathetic, and hardworking nurses: precious, Yeshi, justin, sisay, megan. These nurses go out of their way to help you find comfort and relieve. No matter how often you page them, they show up with a smile and are so patient. However, not all of our experience here was great, which is unfortunate bc we loved some of the nurses and all the OT/PT staff. There were 3 nurses/aides who did not do a thorough job. Two didn't clean my mom at all after removing the bedpan. One of them didn't even put my mom's diaper back on and just left it hanging off her butt. Jeff from PT discovered it when he came to help my mom off the bed to go to PT. Another time, my mom had a stool movement accident while the nurses moved her to the bed and it got on her bed and floor. They didn't wipe her or clean it until I saw it and asked someone to come do it. Another bad experience, two nurses came to reposition my mom in bed and left. After 10 minutes, my mom was in a lot of pain and she's immobile so she can't move and she's heavy so I couldn't move her myself so I called the nurse to help me reposition her. One nurse aide picks up and said with a hint of frustration and rudely, "Again? We just moved her!" Her tone of voice caught me off guard and I responded, "I know, but she is in a lot of pain and needs to be repositioned." I was a little upset at her response bc I thought it was unprofessional. After I expressed my disappointment at that nurse, the lead nurse and charge nurse were very apologetic and had that aide removed from my mom's care. Overall, I would have rated this 4 stars, but the bad experience was too frequent. I do love how they immediately address the situation though. I appreciated that they listen and act asap. I wished all the nurses were as wonderful and not just a few.
