My Mom was a resident at Evergreen Place for 1.5 years. She arrived at Evergreen after suffering a fall at another facility, which resulted in a fractured pelvis. Her apartment was located upstairs and her first day there, she was in so much pain by the time she got downstairs to the dining room that she couldn’t eat. One of the staff members instantly suggested a wheelchair so she could get to the dining room pain free. It quickly became a “comfort” for Mom to be in the wheelchair (she had suffered several falls and was very afraid to walk). The PT came in and immediately told her that she was going to get up and walk and she wouldn’t need a wheelchair. After her sessions with him, and the high levels of encouragement from the staff and other residents, she started walking around on her own with her walker! Mom ended up going to the hospital for a couple of days (for a different reason) and I received a call from one of the med nurses (from her personal cell phone) each day that Mom was away checking on her and asking if we needed anything! The entire staff, and the residents that are at Evergreen are a unique group of people!!! Everyone there is a big family! Thanks to the love and care she received during her time there, she improved to the point that she didn’t really need assisted living level care. Moving my Mom was a very bittersweet decision, because she had improved (and I whole heartedly believe it is because of Evergreen), but it was like leaving a family behind; not just Moms family, but my family as well! If/when Mom ever needs Assisted Living again, I will 100% contact Evergreen Place to see if they have availability! And I can’t thank them enough for all of their support, love, care, and dedication to each and every one of their residents (whom have become like my loved ones) and my family and I!

My mother moved in and was only able to get an apt at the very end of an upstairs hall. She is 97. We requested that she get a closer room as it was too far for her to walk. They said they put her on a list. She never moved up that list in 3 months. We were told by one person when we signed her up that she could be taken down to meals in a wheel chair - but that never happened. We bought her her own wheel chair and she had to move it with her feet. We asked for a 3 day a week shower assist (which was apparently too many times a week). That never happened even after the 90 day meeting. She has bad eyesight and can’t see the menu or activities list. We asked that she be reminded at the time of events. Nope. She was like a lost soul there. The activities you see people online doing are only about 10-12 of the same people that are more mobile. She wasn’t thrilled with the food so we asked to have a meal with her. When we were served in the guest room the plates were overflowing with food. She said she had never had that much food before. The med techs for the most part were just in and out with her drops and meds. There was only one med tech who would at least visit a few minutes with her. We would go to her instead of other staff (nursing) because she knew mom had what might be up with medications. The directors didn’t communicate with the desk employees or the med techs if we needed something done. We brought in a microwave that mom was used to using with her bad eyesight and asked many many times to have the one supplied removed from her room. It sat there the whole time she was there on her kitchen table. We were told by office staff that mom could go to the strong west right next door for a urinalysis (wondered if a UTI). When we never heard results sent to her DR we inquired- and were told that only residents who chose to go with their staff Dr were allowed to do that. When we requested to talk to nursing staff about this -<br>The head nurse did get approval to have them come to mom and get a sample - however it never came back with a result because they spelled Moms name wrong and there was no resident match - so had to be repeated. By that time if mom really had a UTI she might have had sepsis!! Ever added request to help mom was an added price. After the 3 months we had to get her out of there and we’re lucky enough to find a private 6 person place for her - thank heavens

Completely and utterly disgusted with this facility. Every time I visited my grandfather in the memory care unit, the staff was doing their homework or socializing with one another while the residents just sat there. When assistance was needed from the staff, we always received attitude because we're interrupting their conversation. My grandfather's room was so dirty that we opted to use a public bathroom elsewhere instead of using his bathroom. His bed sheets were RARELY changed which we know because we bought him different color sheets and it was always the same sheet on his bed. We discussed this issue many times with the memory care director but as you can see from the pictures the last time i visted my grandfather, they never addressed the issue or made any effort to clean his room. I should also mention, that the bunch of times they sent him by ambulance to the hospital, he was never where they said he was. Two separate times, we spent hours calling around to all the different hospitals in Rochester to find him. During these incidents as well, Evergreen informed us they would not transport him back from the hospital, so we would go to the hospital to pick him up, only to arrive and find out he was already transported and back at Evergreen. If you ever need to speak to someone in the weekend, good luck because there is no supervisor there and its hard enough getting someone to call you back during the week. I seriously wouldn't send my worst enemy here.
