Where to begin... When I first arrived I was at DeBary Rehabilitation center for aapproximately 2-1/2 weeks. I had reservations when I first arrived. I was not happy with the room I was initially placed in. But my wife requested a room change for me and I was moved to the 500 section. Huge improvement. The nursing staff for the most part was very attentive and professional. Same with the service assistants. Jose and Tony were super and helped me out whenever I needed it. Brian, from the Physical Therapy Department was absolutely phenomenal. He went way beyond what he needed to do to perform his duties. Same with his coworker Phyllis. I would highly recommend this facility to anyone that needs both nursing, as well as physical therapy. Thank you DeBary Rehabilitation Center. Edward D.

This place deserves to be 5*. The staff is excellent, the care is extremely good, and the medical staff is also great. There are several gathering areas for patients to congregate, and some activities as well. The communication is pretty good, and most of the time if you talk to people, they can get you answers. The most important thing is the health and safety of patients, and the care they receive, and you'll get it here. In particular the CNA's went above and beyond for the patience. That is not to say there is not some things they could work on. The facility inside is extremely dated. The bathrooms in the rooms which are shared by 4 people are pretty small. Some of the food items could use some improvement, such as the grilled cheese which all of the residents complained about. And the tvs in public areas used regular cable channels which showed lots of commercials with predatory gambling apps, and the like. If my loved one had not been transferred, I was going to talk to them about getting some kind of streaming channel which showed westerns without commercials. Having to be buzzed in in the rehab was annoying, as was having to walk all the way through it if you came after 5pm. The sign for the rehab outside is rotting away. There is that beautiful lake behind the facility; it would be nice if the patients, perhaps with their loved ones, could go and look at it instead of just the highway. These are all things that Debary can work on, but, that is not why you are sending your loved ones. You want them to be cared for and safe, and they will be. You want to be able to call the place on the phone, have someone answer, and get you the answer you need. This is a great facility, and we would definitely use them again. We only changed due to how far it was from our homes.

Debary has been taking care of my mother and unfortunately, I live out-of-state. But the one bright shinning star there at Debary, it has been Lisa Zook she has gone consistently above and beyond to help me with situations dealing with my mother and making sure my mother feels safe there and is taken care of with with staff like Lisa at this facility. It makes us difficult time. For the family's of patients that they are caring for easier

My brother has been in DeBary Health and Rehabilitation for 3 weeks, I visit often and witness the great care he has been given. Shout out to CNA’s Bre and Alesia. PT Bryan and Gina and all the nurses and nurse practitioners! Empathy and compassion has always been shown!

The staff is amazing they treat residents like family and make visitors feel very comfortable, the level of care exceeds my expectations. the facility maintains cleanliness very well. They also have food trucks every now and then, last time I got a latte and seen staff help bring out residents for them to grab something from the coffee truck too, 100% will recommend :)
