Short-Term Rehabilitation

Many people require short-term rehabilitation following a fall, surgery or hospital stay, before returning to their homes. Our staff works closely with patients and their families to ensure the best possible results, which means a smooth transition back to their home environment.

Following an acute hospital stay, Concordia can provide inpatient short-term rehabilitation in one of the several "skilled nursing facilities" in the system. The majority of our short-term rehabilitation beds are in private rooms, making your stay as comfortable as possible.

Our dedicated short-term rehabilitation team is in place to motivate, support and increase the overall functioning of each patient. Our objective is to meet the needs of those individuals with the goal of getting them back to life as quickly as possible.

The clinical staff and therapists work together with our physicians to develop a customized program for each individual. To enhance recovery, Concordia's food service staff provides delicious and nutritional meals and our activity staff provides therapeutic recreational activities for whoever chooses to participate. Cable TV service is provided in every room and many are equipped with free Wi-Fi Internet capabilities.

We encourage our short-term rehab patients to participate in their daily care and to be proactive in their recovery. Our in-house social services staff is available to meet with patients and their family in order to stay informed of their progress and discharge plans.

Personal Care

Personal care is inpatient care where residents receive professional assistance with activities of daily living, such as bathing and personal hygiene, mobility assistance, wound care, preparation of special nutritional meals or diets, housekeeping, activities, laundry, spiritual care and more.

With over 850 personal care beds in the system, Concordia is one of the largest nonprofit providers in Pennsylvania and the tri-state area. All of the facilities at our personal care locations feature professionally decorated rooms that can be furnished with personal items, dining rooms to enjoy the home-style meals and several lounge areas for daily activities.

Concordia has an outstanding personal care staff that undergoes a rigorous screening process, receives ongoing training and is dedicated to each persons well being. We provide 24-hour nursing support and supervision as well as physician services.

Volunteers and fellow residents offer companionship every day, while the activities departments organize trips and events, as well as classes for the residents. Chaplains hold regularly scheduled worship services and Bible classes.

Long-Term Nursing Care

Individuals in long-term nursing care typically require a higher level of around-the-clock care from various professionals - including a medical director, nurses, therapists, social workers, dietitians and many more.

There are several skilled nursing facilities in the Concordia system that provide long-term nursing care. The majority of our long-term nursing care beds are in private rooms, making your loved one's stay as comfortable as possible.

All of Concordia's long-term nursing care facilities have spacious physical and occupational therapy gyms with modern equipment available in conjunction with hands-on therapy treatment. The clinical staff and therapists work together with our physicians to develop a customized program for each individual.

My mother has been a resident for a year. She is treated like family and “loves the food”. Care takers, nursing , administrator, and hair dresser all work as a unit to provide residents a happy, comfortable and safe environment . Thank you, Concordia for providing a cozy, safe, And compassionate environment for my Mother’s last years of her life. I am impressed by Concordia’s personal approach, caring employees and the excellent condition of the property. I would not hesitate to recommend Concordia at the Orchard to anyone who is looking for a home for their family member who is in need of additional care and is no longer able to live on their own. I got to experience firsthand the array of activities and events offered to the residents when I stopped by to visit and there was a bingo game or crafting event happening. It’s really special to know they offer an abundance of activities to improve your loved one’s quality of life.<br>Concordia at the Orchard gave our family peace of mind, knowing our Mom was cared for in the most excellent way as we all visited there numerous times throughout the week and never once had to question the standard of care she was given. Any issues were always addressed to us promptly as well. The communication was superb.

My mother moved into Concordia at the Orchard about 18 months ago because she was no longer safe to live on her own, yet she didn’t want to live with any of her children. She was initially upset about having to move to an assisted living facility but has since said that it was the absolute best decision she/we ever made. To say that the Orchard is “her home” doesn’t say enough. Every single staff member at the Orchard knows my mom by name, but they know so much more than just her name. They know how she’s feeling each day, they know what she likes (and dislikes) to eat, they know what activities she prefers to attend and so much more. My mom has gained an extended family at the Orchard that we could have never imagined. As an RN myself, I value the open communication between the staff, physician, and family. When they arise, mom’s medical needs have been addressed quickly and effectively each time. We couldn’t be more happy with Concordia at the Orchard!

Choosing a facility for a loved one can often be stressful and very difficult. My family found ourselves in this situation due to a life changing need, but the staff and administration at The Orchard were there to support and facilitate the transition from home life to assisted living.
