I have not been here visiting my friend since the new owners took over. What a difference!!! The staff seem friendlier, the areas look so much cleaner, and I'm so impressed! The turnaround is so noticeable. Please accept my congratulations on this amazing change and keep up the good work!

If I could give 0⭐, I would. My mother came home from rehabilitation this last week. The last 4 weeks of her rehabilitation here (other than PT and OT) is absolutely horrific. My mother was admitted last minute on a Friday night, and she wasn't fed for the first 2 days because no one let the kitchen know. We attempted to address it after the first time, but without being on top of them, it doesn't get done. My mother was left in bed for extended periods of time, and we have caught several nurses and CNAs blatantly lying when we would attempt to call them on it. Her medication for her pain was never administered, because according to them, she never asked, but my sister and I would come to bring her clothes and she's crying over pain. They usually forgot to put in her dentures most days, and when they did remember, they'd usually forget to take them out at the end of the day. I've worked in the medical/insurance space for a long time, and the management transition to Civita was the excuse every time is absurd. If you know you're in a transition, you adjust your team to accommodate. Now, because of all the fumbling on their part, my mother now has nothing for her pain management, and between the facility and the pharmacy, they made the decision to not fill her medication, and I'm assuming it's because they think we're drug seeking. My sister and I will be filing a report with the state, as well as the program with her Medicaid regarding the dangerous situation my mother was in. The weekend Supervisor (because there's not a single nurse there on the weekends, which is a disaster waiting to happen all on its own) then tried to blame us for their shortfalls. At one point while visiting, we heard another family arguing with a staff member because their loved one was left in bed for days and they were trying to lie about it to cover their asses. They're going to end up unaliving someone.

Once my father was moved to the hospice room at the facility, he was treated with the utmost care. It was a tough time for six days with him being so ill but the staff there especially Wayne the nurse was amazing and so was Kate the social worker. They made the transition from life to death with my father that much less painful. They were extremely compassionate. The people at the front desk did a wonderful job too.

First time I was @ Salmon Brook Rehab. to visit a loved one and I was pleasantly surprised the staff was immediately attentive & helpful. Charity is the name of an excellent aide & Morgan excelled in keeping patients floor clean. These two employees made a comfortable & reassuring feeling when a loved one is in Salmon Brook Rehab & Nursing Center.
