My Mom needed to go to a skilled nursing facility just for a couple weeks while she got her mobility back after a hip replacement surgery. While at the hospital mom was presented with a list of facility that acted like a hybrid facility for rehabilitation. These facilities clearly had fake star ratings.... I had urged my mom to wait until I could drop by the facility. I did get to drop by before my Mom made it there. I dropped by unannounced because I wanted to see what this facility was really like? The lady at the front desk was rude and told me not to move as she ran to the back of the facility as if there was something to hide? Another girl came up who was very nervous acting, she said normally they schedule tours. I told her I wanted a simple tour now! What I witnessed was awful. The facility itself is very outdated. Those there did not look very happy....the common area where the dinning/Tv room had nothing to do in it, the TV was off and everyone was just sitting around not engaged at all. I was shown a game cart that hardly had any games on it and they looked like they had seen better days. I asked about movie nights or any social events? I was pointed to an old popcorn maker sitting in the corner that did not look like it was used often. I then asked to see where the physical therapy took place...? I was lead to this old room with hardly anything in it. It looked like it was from the 70s, no real equipment...I cannot imagine how anyone would get PT. I asked do the patients have TVs in their rooms? I was told that some rooms have them and some don't that some just bring their own Tvs. I was shown the outside area where smokers could sit outside or if anyone wanted to walk around outside....it was scary looking and not inviting. There was fast food trash sitting on a table near the foyer area. The nurse staff all looked irritated to have anyone looking around and I was not even shown down any halls or rooms. So, My mom stated she would just see it, though I advised her that this was not a good idea that the facility was in poor condition. My mom did show up and she turned around and left! I got a call from the coordinator that I had spoke with on/off that day. She told me my mom had left and I told her I was well aware and approved! She asked me what was wrong? What could be done different? I told her my experience from when I 1st arrived. She ghosted me after that. I am writing this honest review to hopefully help someone's loved one from ending up in this terrible place. If you are even considering it...drop by unannounced toward the evening. You will reconsider...I know it!

I am a Hospice volunteer and visited a patient there today. This facility has had a major upgrade since I visited someone here previously. It’s very nice, clean, quiet, and accommodating. I was very impressed.

My wife was supposed to stay here for 5 days of respite care. She is under Hospice care, so I talked to the Dr, who said comfort is most important, and ordered that she could have her cigarettes and one drink per day. On the second day, she was burning hot with fever. I notified staff, hoping for the best. I picked her up on the third day to take her home due to dissatisfaction. When they brought her belongings, all of her cigarette packs were unopened, as were her beverages. I don't care what they think is best, when there are doctors orders to make her comfortable while she's dying, they should follow those orders. I would never, and WILL never put a loved on in this facility again, even for a few days. The staff was courteous, and friendly, but that is negated by the refusal to follow doctors orders.

Azalea Heights has taken such great care of my mom when she has stayed in their care for short stays. My mom is happy and enjoys the activities provided while she is there. Everyone is welcoming, friendly, genuinely caring for the residents. The facility is clean and safe. When I call to check on mom they are always available to talk to me and provide me detailed information and that means a lot to me so I can be assured and at ease that my mom is being cared for the way I would care for her. Thank you Azalea Heights!
