We cannot say enough about the incredible care Transitions Care out of Champaign, IL provided during our grandma’s passing. From start to finish, they made what felt impossible not only manageable, but peaceful. Nurse Amy went above and beyond in every way. She gave us absolutely everything we could have needed and more. She educated our entire family, helped us feel confident that we were doing the right thing, and gently taught us what signs to look for, how to safely move Grandma in her bed, and how to care for her in those final days. She even brought up things we didn’t know to ask about and made sure we had every piece of equipment we needed. When the time came, Amy came after hours to ensure we were not alone. That meant more to us than we can ever express. Latarika, another nurse who visited, was just as compassionate and kind. She shared personal family memories that helped us feel understood and connected, like she truly related to what we were going through. She even reached out after grandma’s passing to express her condolences. Randy, our CNA, brought such calm and comfort into the room. He helped everyone feel at ease as Grandma passed. Maddie, who began the intake process, set the tone from the very beginning with professionalism and warmth. Because of this team, Grandma did not have to suffer. They handled everything so our family could focus on being together and grieving without distraction. We truly couldn’t have asked for a better experience. We will recommend Transitions to anyone needing their services. They made an incredibly hard season feel supported, peaceful, and even joyful in honoring our grandma’s life. While we may have lost a family member, we also feel like our family expanded as we welcomed in these Transitions staff members into our hearts.
