This two star review comes from a mix of good and not-so-good experiences over a span of 8 years with both of Scott's parents having spent significant time here. The good - The rooms and common areas are generally nice and well laid out. - There are individuals that are friendly and responsive - particularly the social worker, many of the floor nurses, some of the aides, and the dietician. - Scott's dad actually received quite good care in the 2018 and 2019 timeframe. The not-so-good - In recent years, therapy and nursing leadership has been deficient at best. - There was one particular meeting regarding Scott's Mother's care in which there had been months of at least weekly communication about several ongoing care issues. The head of nursing joined the meeting and claimed this was the first he had heard of any of the issues. It is really impossible to solve problems or improve when there is a leadership culture of gaslighting and denial. - Over the past several years when Scott's Mother was a resident at the Birches, the result of this culture manifested itself repeatedly. - As her condition deteriorated, there never seemed to be any reasonable idea or plan for how to adjust. When she began to have difficulty eating, the recommendation from the medical, nursing, and therapy team was to move toward swallowing studies and likely a feeding tube when it was obvious to family and others who where with her regularly that a simple dietary change is all that was required. - This extended to an inability to solve even the most basic problems, like wheelchair fitting (we eventually gave up and bought our own for her), broken or missing wheelchair footrests, a broken soap dispenser that was never fixed over 2+ years, frequent confusion over where/how to get medication, etc... - One of the five star reviews was left by the current Birches administrator (just saying...) - Another recent review mentioned having residents just use their depends rather than the toilet. We also witnessed this. This became much more common in our experience after the assisted living facility opened in late 2025. It seems that staffing was further diluted at that point. Other issues, like wait times for call lights also seemed to become worse at the same time. We saw wait times approaching an hour at the end. - Many of the staff have limited English language capabilities. This becomes an issue when communication with residents is required or when instructions written in English need to be followed. There were many examples of meal instructions that were just completely ignored (which seems preposterous until you realize that some of the preparers/servers probably can't read them). All in all, I couldn't really recommend the Birches Health Center in its current state due to all of the quality of care issues. That said, it is a nice facility with some individuals who really care - so with the right leadership, I think there is at least some hope for improvement.

About a month ago, my grandma was sent here for rehab after hip surgery. We were unimpressed with the care she received and would not recommend this place. For one, the staff were rude and unresponsive, and they did little to go above and beyond when caring for my grandma. When my grandma signaled for help to use the bathroom, the staff would often take more than 15 minutes to come and assist. When they did assist, they would often leave her alone in the bathroom, which was extremely unsafe to do because she has Alzheimer's and forgets she can't walk on her own. They would also react rudely when my grandma realized she did not actually need to use the bathroom. It is very common for Alzheimer's patients to have bathroom anxiety, but regardless, it is unprofessional to chastise a patient for utilizing the call button. The biggest red flag was that the staff lied about my grandma's care, forcing her to be released before she was ready. They claimed that they showered her every day, but she went two weeks without a shower. They claimed that they weighed her every day, but she hadn't been weighed since her first day there. Most importantly, they claimed that she was able to walk stairs before anyone had ever seen her do that. She was supposed to be there 6-8 weeks, but she ended up being released on week 4, still unable to walk properly without a walker and unable to walk stairs on her own. Between the inattentive care, rude staff, and fraudulent practices, this place is the epitome of unprofessionalism at best and a scam at worst. Do yourself a favor and send your grandparent somewhere else.

The only reason I'm writing this review is because I wish someone could have warned my family before we trusted this recovery center to help my Grandma. I'll try to keep emotion out of it & just state the facts: 1. They treat pain medicine "As needed, & upon request only" but they don't tell you that upon your arrival. My 86 year old grandma went 16 hours without any pain medicine just 2 days after a highly intense (I'm talking metal plates drilled into her femur) surgery. 2. When we learned of this & began calling every 4 hours to ensure they gave her the pain medicine on a routine while she healed. We would call & their delivery time to actually give my Grandma pain medicine would be as long as an hour. 3. When I was visiting, I witnessed my Grandma wincing in pain from trying to move. It was 2:30, so I asked the nurse if she received her 2:00 pain medicine, & they responded with "She doesn't look in pain to me." instead of checking her records & dismissed me. 4. Because they were not giving her pain medicine on a regular basis, even after we were calling every 4 hours request this, my Grandma was unable to put weight on her legs, & they instructed her to pee in her depends. This happened the ENTIRE time she as there. Mind you, my Grandma had just had this surgery, her incision was near her groin area, & she is already prone to UTI's & Bladder infections that further confuse her memory. 5. On one of the many visits I made to see my Grandma, she was sleeping in bed upon my arrival around 6pm, with soiled depends half hanging out of a small trash can, completely opened beside her bed. I do not know how long it was there. When I arrived my Grandma woke up & was talking with me. A staff member came & delivered her dinner, & did nothing to clean the garbage or other random food items throughout her room. Please keep in mind, my Grandma literally was not walking at that time due to the pain she kept experiencing, & even with pain medicine, couldn't walk or get up without assistance, so any food items randomly placed around/in her room were from staff. This included an old glass of milk. Old milk, & soiled depends. Quite the appetizing mix for her dinner ambiance. 6. As stated above, my Grandma was told to use her depends as a means of peeing while she was there. In one occurrence after peeing in her depends, the nurse came to change them. The urine had gotten onto my Grandmas clothing, including her socks. Instead of refreshing her clothes, offering a bath, literally anything to bare minimum CLEAN my Grandma, the nurse changed the depends then re-put on all of the soiled, damp clothing. My Grandma said "I think my socks got wet" as an ask to please change them, & the nurse looked at her, & yanked the damp with URINE sock up aggressively on her SURGICAL LEG. 7. On the last night my Grandma was there, she called me & my parents in extreme confusion. She didn't know where she was. She thought she had driven herself home & that someone was in her house. She was terrified, had peed herself, & was calling for help. My mom called the nurse station for help on a separate line & no one answered. This was at 7:30pm. My mom then instructed my Grandma to call for a nurse on the remote attached to her bed. We were calming her down on the phone til a nurse came. We explained to the nurse what was happening & she said "Okay." then heard her tell my Grandma "You are at the Birches at Trillium Woods." My grandma told us she then turned all the lights on, took off the covers on my Grandma, & left the room. Turning on the lights, taking off any covers to expose my Grandma had wet the bed in fear, & leaving without any reassurance, consoling, or re-orienting. Incredible care for a 5 star rehabilitation center towards an 86 year old vulnerable adult having a literal mental crisis. She went to the hospital that night in extreme pain, calling out "Why wont anyone help me". The next day at the hospital, She was up doing her rehab, going for assisted walks, was laughing with the nurses & even getting to use the toilet! Imagine that.

Skilled nursing facilities can really be hit or miss—and it’s honestly a little scary to trust one with your loved one’s care. But we felt confident that my mom was in good hands at The Birches at Trillium Woods. From the very beginning, it was clear that they were attentive to her needs and truly cared about her comfort and well-being. I feel strongly that this is an excellent facility. The staff genuinely care about their patients and are committed to providing thoughtful, compassionate care.

As a result of a terrible fall my bride lost the ability to control her legs and broke her right humerus which resulted in having no shoulder on the right side--her dominate side. A short stay in the hospital and then to a transitional care center for rehabilitation and healing. We were fortunate, no...blessed, to have found a space at The Birches at Trillium Woods. From the moment we arrived we found The Birches to be a welcoming place, a respectful place, and a healing place. There is a very obvious culture of care, concern, and healing at The Birches. It shows in every interaction with every level of staff. Regardless of position or job description everyone who came into Barb's room always greeted Barbara with a friendly greeting from a friendly face and always addressing Barbara by name. At The Birches one is never 'she', 'he', 'her', or 'him'. Everyone has a name and The Birches' staff respectively uses it in all their contacts. All staff did their assigned tasks so well. Clearly there was intensive training. Additionally, the staff were all so friendly in professionally, personal ways. Barbara, because of her injuries, felt very fear- filled and vulnerable. In a very short time she felt very confident and safe with the staff members. She knew every staff member wanted one outcome for Barbara: "Get stronger, heal, and get to go home safely and soon". The culture of healing and respectful treatment of each resident was like a mantra every staff person recited everyday in so many ways. Barbara and I benefitted from this "place of healing" in so many ways. We have gained so much respect for all the people of The Birches at Trillium Woods...truly a very special Place of Healing.
