About Stone Oak Care Center

Stone Oak Care Center is a skilled nursing and long-term care facility in San Antonio, Texas, licensed for 152 residents. The community serves patients requiring rehabilitation after hospitalization as well as those needing ongoing skilled nursing care, including memory care units.

Families consistently praise the facility's administrative and care staff, particularly noting the welcoming approach of staff members during admission and transition. Multiple reviewers highlight attentive care during recovery stays, regular check-ins, and attention to cleanliness and medication management. Activities such as bingo, crafts, and community gatherings appear regularly in accounts of daily life. One family wrote, "they're keeping him and his room clean" and checking in frequently. Some families also note the freedom residents have to move around common areas and access snacks and water, which they value especially in memory care.

However, experiences are not uniform. Some reviewers describe significant concerns about memory care operations, reporting locked outdoor access despite stated policies, minimal staffing on certain units, and lack of structured activities. A small number of families raised serious concerns about care quality and staff conduct. These conflicting accounts suggest variable experiences depending on unit and timing.

Stone Oak holds a 4.2 out of 5 rating based on 249 Google reviews, with particular strength in reviews praising administrative support and transition care, though some lower ratings flag operational and care consistency questions.
