Would never recommend placing a loved one in this facility. So much management turnover in a short amount of time is a big red flag for me. The Activities Dept. was one of the only redeeming qualities this facility had, then their two best Activities staff left last winter and it all went downhill from there. I'd give this place zero stars if I could.

My father stayed at St. Anthony Park Home for 25 days, and while I want to acknowledge that the nursing staff were compassionate, professional, and truly caring, there were several serious systemic failures that overshadowed his stay. My father was placed in a secluded corner room, which contributed to isolation and limited visibility from staff and visitors. Given his condition, this placement was inappropriate and did not support his safety or well-being. Communication throughout his stay was consistently inadequate. As his primary contact, I did not receive timely or clear updates about his condition or care plan, which caused unnecessary stress and confusion. I also had significant difficulty communicating with the kitchen manager, who failed to return multiple requests and only responded after I left a very firm voicemail. This lack of responsiveness is unacceptable, especially when dietary needs are involved. The discharge process was deeply concerning. The social worker did not clearly explain my father’s discharge plan, failed to follow up on services he may have qualified for, and provided no information about a bath seat, a basic and critical safety item for his return home. Most troubling, shortly after discharge, my father fell and was rehospitalized. Given his weakened condition, this outcome was foreseeable and raises serious concerns about whether appropriate assessments, therapy, and safety planning were completed prior to discharge. I am also concerned that he did not receive adequate physical therapy during his stay and was discharged before he was truly safe to be at home. The nurses who cared for my father deserve recognition for their dedication and kindness. However, the lack of communication, poor discharge planning, and insufficient rehabilitation support are serious issues that need immediate attention. Families deserve better transparency, coordination, and safety standards during such vulnerable times. Edit: Following these events, the administrator, social worker, and Director of St. Anthony Park Home did reach out to me in response to my concerns. While I appreciate the follow-up, the issues were addressed only after my father’s discharge and subsequent rehospitalization, at which point the impact of these systemic failures had already occurred. With that being said, please think twice before you allow your loved ones to be cared for at St. Anthony Park Home.

I stayed in this facility for 3 weeks last September(2024). I went there for rebilitation. I would recommend this place always. The staff were amazing. Thank you all so much!

The quality of care at St Anthony Park Home has seriously diminished since it was sold to a multi-facility company. There are still some caring employees working there and doing their best to provide compassionate care but the overall quality of care has degraded to the point of neglect in some instances. If you have a relative in this facility, pay close attention to what is going on with them. This is no longer a facility you can trust in. Previous Review when privately owned; My mother in law has been living at St Anthony Park Home for about a year. Her health improved vastly after becoming a resident here. The staff is kind and attentive. We wish we were able to care for her in our home but are thankful to have found a nursing facility that we can trust. This is a small privately owned facility and the owner is on site and responsive.

Not clean at all. The staff is not friendly. I walked in and wanted to die. Everyone besides hospice nurses are unpleasant and untrained. This place is unacceptable. If you hate your loved one send them here if you really care for them please send them somewhere else. This place needs to be looked into.
