Do not send your loved ones here, this place is outright deplorable. Do not get me wrong, there are some staff members who do care, and make sure patients get what they need when they need it, but the majority of the employees here do not care about the patients or treating them as human beings, never mind giving them the proper treatment and attention they need. Medications are often late, missing, or even mixed up, again, absolutely inexcusable behavior from a health care facility. Some staff are incredibly rude to the patients and each other, and will argue amongst themselves loudly in the halls, I've watched with my OWN EYES CNA's enter rooms to assist patients with bed pans, and afterwards dump and rise them IN THE PATIENTS SINK! Bed pans which are SUPPOSED TO BE DISPOSABLE! Of course, they didn't disinfect or clean the sink afterwards. After complaining to management, I've yet to see the bed pans being cleaned in such a manner since, however that happening AT ALL is disgusting and speaks to the lack of care that the staff have, and I guarantee if it wasn't such a massive health code violation, management wouldn't have changed anything. They do not move, adjust, or help clean patients who are immobile and are prone to bedsores, only acting when the sores become so bad they require hospitalization. The staff don't even both assisting patients by simply pushing their wheelchair into the bathroom for them, instead offering patients bed pans, diapers, or pee pads because it's less effort for them to let patients soil themselves than it is to simply assist them into their wheelchair, or to push them into the bathroom. A few of the bathrooms have 1inch tall "ramps" or "speedbumps" in the doorway along the floor, making it difficult and DANGEROUS for patients in wheelchairs to attempt to enter the bathroom, and as I previously stated, many of the staff will not bother assisting patients themselves. They would rather their patients lay comatose and silent in bed all day without bothering the staff. I've witnessed a woman decline from using a walker, sitting up, talking, to laying in bed, screaming in agony that she wanted to die. When asked multiple times if EMS should come to assess her, staff insisted it was just muscle spams, leg cramps, etc. After a member of administration was laid into regarding this woman's lack of treatment, the woman was taken to the hospital and it was discovered she had an internal infection, she would've died if not taken when she was. To the staff that care, and pour your hearts into your jobs, thank you for all that you do, you truly are guardian angels and you have no idea how much your effort means to these patients. You genuinely are saints. To the rest of you, I have no idea why you're even in this line of work.

Not much to say more than excellent, never had a bad experience, everyone there was great,the head nurse on my floor shared here lunch with me and the mighty quinn, my drill sergeant who taught me to walk again, iam truely grateful

Do not send your loved ones here if you care about them at all, because they won’t. The staff are absolutely HORRIBLE to the patients. They let my disabled grandfather sit in his own urine and feces for 3 hours and told my grandmother that it ‘wasn’t their job’ while they sat behind the desk chatting and literally eating cake. There are 1 or 2 staff members that care but this place should be shut down. They only agreed to test him for a UTI and do physical therapy with him after threatening legal action. This is literally a rehab health center, what is their job if not these things? Shame on the staff, karma will come back when your loved ones are in one of these places being treated this way.

I was on the 3rd floor for 3 weeks. They're very under staffed. Most of the aides were very rude. They left me in soiled diapers, never gave me insulin for my diabetes, check blood sugar when they felt like it. The food is nasty. If I could I'd give them - 100 stars. The only good thing was PT/ OT therapist. This place needs to be closed down. I wouldn't recommend this place to anyone

My aunt was a patient on the 3rd floor of this facility. I have nothing but positive things to say about her experience there. The facility itself was very clean. The rooms were comfortable. The food was ok, but served promptly and the patient had options for every meal. The biggest plus was the staff. Almost every employee on the 3rd floor was sweet, kind, attentive and gentle to my aunt during her last month of life. Even offering us a private room for her final night so that she could have some privacy and so that I could stay with her until she passed. Was everything perfect? Probably not...but the things that mattered, kindness, respect, and caring made this very difficult time much easier for my aunt and our family. Thank you to all the staff at Sixteen Acres Health Care facility. You have a difficult job and you did it very very well.
