I worked hard to get my dad here from out of state because of the numerous recommendations regarding Shiloh from the medical professionals where he was hospitalized. He spent roughly four months at Shiloh before going into hospice care. From housekeeping to Shiloh’s director, I witnessed the best meals and care possible for dad, even when he was extremely difficult to work with due to his dementia. Their team of activity directors always had special plans for the residents. He was treated like family, and I just can’t imagine him getting better care. This review would end up as a 20-minute read if I mentioned all the outstanding workers by name. I’m all about communication and Shiloh didn’t disappoint. My family is so thankful for the loving attention my dad received here!

Love Shiloh ❤️ The staffs are wonderful and caring, they really make everyone feel at home. I also love that they have doctors who come every week and a nurse practitioner there Monday through Friday. It’s nice knowing residents are well taken care of and always have support when needed. Everyone is super friendly and helpful, definitely recommend!

If you actually care about your loved ones, you will not send them here. I had no idea the conditions that I would be facing when I went to see my grandmother here. I know that no facilities is perfect. However, the staff here are incompetent. There isn’t a doctor on staff. A doctor only comes there once a month. My grandmother had just gotten out of the hospital after a month and needed real care. 2 days after she arrived there, I sent to see her around PM at night and got there to see food all over her while she was asleep. Her tray had not been removed. She didn’t receive any assistance helping her eat, even though she could not eat on her own. She was laying there with food all over her. She had blood under her nails blood all over her face. It took a long time to clean her up. She also needed to be changed, and no one had assisted her. To wake her up, the nurse would just yell at her or push her arm, which was already severely bruised and in pain. They completely ignored her. Mostly, I would hear about nursing staff ignoring certain patients and trying to create a lot of drama. One night I called my grandmother’s cell phone and she answered the phone at night and the nurse picked up the phone and hung up on me. I have nightmares about this place. The staff could not have been more uncaring when she passed. I didn’t receive any condolences from anyone there or any basic decency. She would’ve been better off in the hospital. I know that many facilities are not great but nothing is it as bad as this place. Please don’t believe these fake reviews. This place is an insane asylum. Dirty. Horrible staff. Devastating monstrosity.

Review written on 1-31-2026: My wife was referred to Shiloh Nursing and Rehab after a four day stay at Mercy Hospital; the referral was for 30 days. She found the 700 hall staff to be friendly and helpful. The Physical Therapy team did a good job. Our insurance gave a 10 day approval, followed by a 7 day approval and a 2 day approval. The 2 day approval ended on December 24th. I spoke with a social worker on the 23rd and was told that the Physician would give us a 48 hour warning of discharge or would request additional time from our insurance. I hadn't heard anything by mid morning, on the 24th, and drove to Shiloh to talk to the social worker. She told me the Physician had written a "peer" letter that was submitted to our insurance. I mentioned a few more days would benefit my wife. The social worker said she felt it was unlikely that our insurance would approve more time but that she would be able to stay until the 26th or the 27th. At 3:00 pm, a Shiloh nurse called me to tell me that the insurance had denied more time and that my wife would be discharged on Christmas Day. I reminded her that Federal Law required them to give us 48 hours notice of discharge. She acknowledged this but said my wife would have go home by midnight on the 25th. Since I knew there were no decision makers at the facility at 3:00 pm on Christmas Eve and since I knew she wouldn't get any Physical Therapy on Christmas Day, I went brought her home. A few days later I called our Insurance and was told that they denied the additional time since no progress notes or clinical notes had been sent. They did confirm that her approval was good through the 24th. They also told me they had faxed Shiloh , that morning, reminding them that they had not received any medical documentation for the 24th through the 31st. I called Shiloh and asked to speak to the Administrator/Director. I was transferred to one of the lead nurses. She looked in my wife's record and told me that their Ft. Smith office had called on the 24th and told them that Cigna wasn't going to pay past the 23rd. I told her that the approval was good through the 24th. She said she would check into this and call me back; the call never came. Roughly 10 days later I wrote a three page letter to the Administrator; I still have not been contacted by anyone. A Mercy Home Health Social Worker told us she had worked in nursing homes for several years and odds were that the key staff didn't worked on the 24th. That the Physician could have written the peer letter we were told about but it was probably sent to the wrong place. Apparently, having an authorization that must be renewed a day or two before a major holiday can result in you getting sent home even if you would have been approved for more time. I took away one star for the disappointing way she was discharged. 2-2-2026 - I am amending this review. Today the administrator contacted me; she had been on leave since the first of the year. She apologized and thanked me for the feedback in my letter. She agreed that the timing of my wife's insurance authorization and the holiday was the likely reason for the was she was discharged. If we needed non-acute care rehab we would consider Shiloh again. If the rehab stay intersected with a major holiday, I would be very proactive about the insurance authorization, hoping to prevent a repeat of what happened on Christmas Eve.

My mother and father are both at Shiloh health and rehab. They receive the best care I could hope for, and seeing them smile and happy is the best feeling. The care is amazing and you can tell they really care! The food is great, they ensure they do activities that they enjoy. The communication is great as they keep me updated on everything. I couldn’t ask for anything more.
