Nursing & Rehabilitation Centers

Our skilled nursing services consist of an interdisciplinary team approach focused on the individual needs of our patients and residents. At Capital Health Care Network we pride ourselves on delivering quality care each and every day to those patients and residents we are privileged to serve.

Many residents come to us for a short stay following an illness, injury or acute hospital stay, while others require ongoing, quality long-term care and support. Through our health centers, we provide 24-hour skilled nursing services and comprehensive rehabilitative services.

Hospice

We are proud to offer hospice services as part of our continuum of care for families and individuals facing end-of-life. Our goal is to sustain quality of life by providing medical care, pain management, emotional support and spiritual guidance during the time leading to and surrounding death. Hospice is about caring not curing.

Our interdisciplinary team includes:

Hospice services include:

•    24/7 availability / nurse on call
•    Pain & symptom management
•    Prescriptions, medical supplies & equipment related to hospice illness or injury
•    Assistance with social, emotional, and spiritual aspects of dying
•    Education about what to expect
•    Coaching caregivers on how to care for patient
•    Physical therapy as needed
•    Short-term inpatient care as needed or if caregivers need respite
•    Bereavement care and counseling to surviving family and friends
