Rehab Without Walls San Marcos provides post-acute neuro rehabilitation and residential care for adults with brain injury and other neurological disorders in San Marcos, Texas. Since 1978, the community has served individuals requiring more therapy, supervision, or physical care than can be provided at home, bridging the gap between hospital and home settings.

The community offers two residential care programs: transitional residential care with active treatment from 7:00 am to 9:00 pm daily, and a Supported Living Program providing 24-hour supervision for individuals with extended rehabilitation needs. Customized neuro therapy programs incorporate each individual's goals and interests, beginning in clinic settings with a fully equipped therapy gym and private treatment spaces, then transitioning to community settings such as local parks and grocery stores as progress is made.

Treatment includes cognitive rehabilitation addressing memory, attention, organization, planning, and problem-solving skills, along with social skills training. The interdisciplinary team provides physical therapy using modalities such as ultrasound and aquatic therapy, occupational therapy with a holistic approach to environmental adaptation, speech-language pathology, music therapy for sensory and cognitive recovery, neuropsychological assessments, vocational therapy, and counseling services. Weekly medical and health team visits and monthly outcome goal plan reviews are part of the care model.

The Supported Living Program emphasizes maximizing quality of life by encouraging leisure, social, and vocational pursuits that are meaningful and productive. Staff foster autonomy and personal growth through individually tailored programs, with functional activities promoting independence in self-care and meaningful interpersonal relationships.
