About Raleigh Rehabilitation Center

Raleigh Rehabilitation Center, located at 616 Wade Avenue in Raleigh, North Carolina, is a skilled nursing and rehabilitation center that provides both short-term and long-term inpatient care. Patients may be admitted following a qualifying hospital stay, transfer from an assisted living facility, or from home with a physician's admission order.

The center offers 24-hour nursing care along with physical, occupational, and speech therapy, available up to seven days a week for short-term rehabilitation stays. Patients may also arrange respite in-patient stays ranging from a few days to several months for permanent in-patient needs.

A dedicated Recovery Program is available for individuals who were hospitalized with COVID-19 and are now in the recovery phase, offering a supportive environment to address both rehabilitation and nursing needs as patients work to return to their prior level of function.

The center has added specialty clinical programs intended to better serve patients, which in some cases includes additional staff training on specialized equipment and treatment programming to support recovery. Portable telemedicine equipment is also available during the day, after hours, and on weekends, allowing residents to consult with providers without leaving their room.

As part of its infection control efforts, the center has worked to vaccinate its healthcare staff against COVID-19 and follows guidance from federal, state, and local health authorities, including providing information to residents, families, and the public about any new COVID-19 cases identified at the facility.

If I could give zero stars I would!!! Do not send your loved ones here!!!!! My grandmother was not even there 24 hours and she almost lost her life. This morning my mother was on the way to the center and when she got there my grandmother was unconscious, the staff did nothing and if my mother wasn’t there things could have gone another way. My grandmother told the staff she could not breathe multiple times (she told us this when she came to at the hospital)and they did nothing!!! When mom said she’s calling 911 and asked what happened they kept telling her no please ( they must get in trouble if someone else calls) and they finally called. My mother talked to her 45 mins prior to arriving and she was awake so this happened within a 45 minute span. She couldn’t even get proper care for 24 hours without almost losing her life. DO NOT SEND YOUR LOVED ONES HERE!!!!!! Thank God my mom was there

I spent about two weeks at Raleigh Rehab Center. Overall everyone was nice other than one Aide but I only saw her once or twice. The nursing staff worked with me on my meds and were good overall. The PT staff was excellent and very accommodating for my needs. They really tried hard to get me back walking on my own again. Also worked with OT staff a couple times and they were excellent also<br><br>The rooms are nice and were clean. Now if you can't get up by yourself to go to the bathroom be prepared for a wait. I think the staffing level is one Aide for 18 rooms but they told you that up front. The meals were mediocre and serving sizes were pretty small. One or two meals I had there were not good at all.<br><br>But overall I was very pleased with Raleigh Rehab. They even followed up with me several times after I was discharged to see how I was doing with Hone Health care. I thought they was really nice.<br><br>So I would recommend this facility and would have rated them 5 stars other than the problems outlined above. This is rating for faculty is for rehab and second floor care not for nursing home care on the 3rd and 4th floors.

I had a pleasant stay at this facility. I had some great nurses like Ms.Josette and Mrs. Tianna. Great assent nurses like Ms. Angela my crochet friend, Ms. Deborah, Ms. Mary and many others. Housekeeping Ms. Chiffon kept my room smelling good. I would like to thank Ms Bridget, Ms. Shaunie and Ms. Cathy for their hard work with entertaining and be off service to the residents. They work above what is required. I enjoyed a few residents Ms. Patricia, Ms. Veronica, Ms. Vern.. I will be back for my get back in Spades ♠️ ladies. I hope I am spelling everyone's names correctly. I would give it a 5 but the food was not good for me. They never knew what to send me. If you have a special diet you may consider having your food delivered. Ms. Terry and Ms Tianna were my life savers. They ordered me doordash and I thank you. Ms. Terry even shopped at Wegmans for me. I had great experiences with all the staff in PT and OT even the office staff members. I know everyone's experiences are different but my time here wasn't bad. If I every have to be in rehab again this would be my first choice.

I have been at Raleigh Rehab Center for 2 months. I received excellent care and couldn’t be happier. The entire staff has been so kind and respectful. They have gone out of their way to make sure that I was comfortable and that my needs were being met. They have an excellent team of physical and occupational therapists. The nurses not only dispense medications, they have taken time to chat with me and really listen to my concerns. The CNAs and their assistants have also been so kind and up lifting. Always with a smiling face and positive attitude. The social committee plans an outing every month. Last month we had lunch at Red Lobster, what a treat! They also plan so many fun activities every day. I enjoyed Zumba classes, low impact from a wheelchair, great motivational music! They also have Bingo, Spa Day, Bango music, Piano man, plays the piano and sings, puzzles, arts and crafts, board games, card games, birthday parties, movie of your choice, and church services on Sunday. These are just a few of the activities. Transportation is provided for doctor appointments. G2Go drivers are also very kind and drive safely. This has become my home away from home. These people have become my friends. I will miss them.
