Family experiences at this facility are sharply divided. Some families praise the day rehabilitation staff for treating residents with dignity and naming specific caregivers, Marie, Cara, Karen, Alex, and Felicia, as exceptionally attentive. However, the majority of reviews describe serious operational concerns: persistent communication failures with families, with phone calls frequently going unreturned and no advance notice given before discharges. One family described being released without essential medical equipment, oxygen, wheelchair, walker, with orders placed only after discharge. Additional complaints include inconsistency between shifts, with evening staff described as unresponsive, and incidents of inaccurate medical record documentation by supervisors.

The facility holds a 2.7 out of 5 Google rating based on 51 reviews, with the vast majority of recent feedback expressing frustration rather than satisfaction.

Do not recommend! No way to contact anyone directly, they never (not exaggerating - never) return your messages, did not provide advance notice of discharge, no evidence a pre-discharge meeting was even held, and released my Aunt with no oxygen, wheelchair, or walker - and placed the order the day AFTER she was released, and only after pressure from her assisted living staff. This facility was recommended by the Augusta hospital caseworker - I do not know how any of these people sleep at night.

There are some staff here who are wonderful, but there are definitely many issues. The weekend supervisor Brad W. lied about my mothers medical records and told me she had Lewy Body Dementia as a reason for why she couldn’t have whole food, when I corrected him and stated that was not accurate, he argued with me that was in her medical records and refused to show me what he was referring to. I complained to the director, she stated he made a mistake and felt bad for lying. The next weekend I saw him he said I couldn’t get water for my mother, I had to ask first. The therapists are lovely, but only present during the week, and several of the nurses I’ve spoken with are also caring, but the weekend administrative staff are worthless, Brad W. being unprofessional and a liar.

If I could give a zero rating, I would, she was only there for rehab .....it was no communication, understaffed, these cna and some nurses were horrible, they do not care about their clients at all. The so called suppose to b supervisor Sitting at computer with ear buds listening to music while cna over there sitting down running her mouth on the phone. My thoughts are why handle so many ppl with no real help. I showed up every single day to check on my mother. I had to change her and make she was good everyday , they dnt assist her to restroom , nobody shows up to help in a timely manner and one day i told 1 nurse she needs changing , she dissapeared and another day, they left her with no water all evening and all night long , that was the last straw for me , I had to discharged her because when I arrived that morning, she was naked in bed and in pain , she was sitting in urine and feces and her gall bladder tube was not taped up , no cna in sight , i discharged her then call ems to transport her to hospital in which she was dehydrated and got a uti infection, hemoglobin real dangerously low, on top of that she had to have blood transfusion. Nursing facilities is no place for nobody , it even wouldnt be a place for a dog , my mother was not cared for properly and im still furious , check on your loves ones , I will give props to one nurse and her name is Maria, Its never the Place , its the ppl they hired that just there for a check and thats sad.

The day staff on the Rehab side is absolutely amazing. They treat my mom with dignity and respect. I highly recommend this facility for Rehab. To name a few: Marie is an excellent nurse, Cara, Karen, Alex and Felicia so sweet to my mom. The evening shift is absolutely the opposite. No one comes when the light is on. My mom suppose to wear Cpap but they neglect to place it on her face, and don’t try calling after hour because no one will answer phone.

No communication, understaffed. My mother can’t walk and is blind. Her needs are not met in a timely fashion. Communication is horrible! You can’t get through on the phone lines on weekends and evenings. I’ve had no one give me a care plan, mom has been there since December 24th. Phone calls aren’t returned, I had no idea it would be this bad.
