The home was very clean. My sister-in-law is there now. I have been there twice and no one came to check on her. The first time we had to go out and find someone to check her feeding tube, the monitor was beeping when we got there. They did come quickly and was able to fix it. Other than the aesthetics I think it is a good place.

My mom was sent here for rehab and literally every person who walks through her door is super nice and helpful. She’s getting physical therapy and the therapists are kind and supportive as well. She’s eating good. The rooms are cleaned daily. I never want her to leave.

A family member was recently a patient at Pleasant Valley, and we found it to be staffed with very loving and caring people. Any issues or concerns that we had were addressed immediately and handled to the best of their ability. A super big shout out to Aisha who was my liaison there and helped me navigate any issues that arose. The only reason I gave it four stars instead of five was because there were several mistakes and confusion about medication that required a great great deal of effort to stay on top of. Rehabs are not the same as hospitals, and I think the therapy part is where they really shine. However, they continued to work on getting everything correct. We appreciated the time that she had there and the hard work of the staff.

My mother was initially admitted here in September 2025 and was there until she died in November. The entire experience here was not good at all compared to other skilled nursing facilities that she had been to for rehabilitation. Long story short, she took a turn for the worse on November 3rd, 2025. Was admitted to Three Oaks hospice on November 6th, 2025. I was told that she would need to pay room and board for a minimum of two weeks while doing hospice. My mother died 5 days after starting hospice. They charged her account $2400 three days after she died. I was told that I would receive a refund. I have yet to receive the refund and have been given the run around every time I call. I was also never given a receipt or any documentation for the $2400 charge. It is now December 15, 2025, and I have not had any follow-up as to when the refund will be issued. To charge someone for room and board AFTER they have died is wrong, especially when you have been billing THOUSANDS to Medicare.
