My aunt was removed from this facility yesterday to go to the ER. Only because I came there raising hell come to find out they have not been washing her, she have been missing dr appointments, they have been abusing and hitting on her! She has deep scratches on her arms, Stage 3 bedsore, Thrash in her mouth and it looks like fur and orange pulp is growing on her teeth! Workers and nurses have been signing the daily log lying saying they were doing her daily routines but when I asked for the logs no one gives them to me they are definitely covering something up! I was escorted by the cops because I was loud on my phone complaining! I believe they changed the facility name to Sherwood! PLEASE IF YOU LOVE YOUR LOVED ONES PLEASE DO NOT PUT THEM IN THIS FACILITY!!! Was told by a worker if my aunt is not removed she would be dead within a month to move her asap! I would add the photos but do not want to disturb anyone!

I would not recommend sending your love one to this place.The staff is friendly but need to improve on their professionalism.If you ask them to do something, they never follow through and resolve the issue.They kept on loosing my father’s clothes and putting other resident’s clothes in my dad’s closet..I made it clear to them that I washed his clothes and asked them to put a sign on his closet door stating this.They never brought a sign out,I had to hand write one so that my dad’s clothes would stop disappearing.They finally brought one out on his last day over there after 3 and a half months.When I got his clothes home after he was released I discovered that 75% of his clothes were missing.It makes no sense!

I have to get information for my moms Social Security at all the rehabs that she stayed at since 2022 and there is absolutely no way of getting in contact with this place. No phone number.

I have found this facility to be warm, caring, friendly, fun and very attentive. I do highly recommend.
