My mother is a resident in the Memory Care section of the facility. When I visit, I see compassionate and caring staff. I am so grateful for the excellent communication with my family from all levels of staff. I had a meeting today with several of the staff members to discuss some questions I had and left reassured that my mother is being well cared for. The Memory Care section has lovely activity directors and I love that my mother has a different nail polish color every time I visit, indicating to me that she is being pampered. I am also appreciative that the men and women are kept in different sections. The meals look very well balanced. Thank you all at Paradigm for your excellent care of my mother and by extension my family!

Under staffed and horrible management. People with leaky and stinky diapers eating at the same table with you. Second shift leave people in their rooms screaming for help and nobody responds as employees hang out up front together talking together.... check colecters. Day shift workers try hard but need more hey. Food is pitiful. Don't use this facility for yourself or loved ones.

My mom was sent here from memorial and this place is horrible. She waited four hours for a cna or even a nurse to come in she was bunked with another lady who they dropped on the floor and she screamed all night I wouldn’t send nobody here. The restroom was disgusting when you walk into the facility it has a smell do not send no one here

This place is terrible. If could give one star I would. They tell you everything you want to hear before your family gets the but once admitted it absolutely different. My give arrived there Jan.20 from an LTAC, the care he received from this place was awful. Every last person there needs to be fired, the nursing director, respiratory manger, and the director of the facility. It's not enough space on this post for me to go into details of they poorly cared for my child. Let's just say he was admitted into the hospital Thursday Feb.5th because of their negligence. If you love your family member do not sent them to this place and if your love one is there now, move them immediately. Because I garuntee the won't make out of there alive.

I can’t give it a 0 this facility because it doesn’t let me only 1 star. This place is HORRIBLE! DISGUSTING!THIS PLACE NEEDS TO BE SHUT DOWN!!! Do not bring your loved one here my father was here from Dec 22-Jan 8th my father had an amputation can’t walk it’s bed bound and people were using his bathroom smelling like urine and my father was scared of who knows who’s coming in and my father is on dialysis needs no infection around him. There was bugs in his room and bed ceiling.Jan 8th my dad was on the floor unconscious and they didn’t even want to call 911 they said for me to call!!! My dad is in ICU fighting for his life because they didn’t feed him nor kept him hydrated nor changed him and he has major wounds horrible ones that got infected to sepsis the staff is so rude and LAZY they don’t inform you of anything even if you ask them a CNA name India is very rude and didn’t want to properly clean my dad and was rude and arguing with me very unprofessional please don’t come to this place !!!
