Memory Care

The Gardens at OakView provides a secure and comfortable living environment specially designed for those with Alzheimer’s disease and other forms of dementia. Our dedicated memory care neighborhood provides a safe, home-like setting, where residents can relax and feel at home. Family participation is always welcome and we work with you to ensure the best experience for your loved one while providing peace of mind for you.

Skilled Nursing

OakView Health Center is designed to provide a full range of services to meet the individual needs of our residents. Our highly trained staff provides around-the-clock specialized nursing care including IV therapy, feeding tubes, pain management, wound care, and rehabilitative services. The team at OakView includes registered nurses, licensed vocational nurses, and certified nursing assistants who work together with registered therapists, rehab technicians and dietitians to provide customized care for each resident.

Rehabilitation

Physicians often refer their patients to OakView for rehabilitation or post-acute care when recovering from a stroke or from orthopedic surgery, such as a hip or knee replacement. Our patients are assisted in their goals for recovery through the efforts of an interdisciplinary team of caring, experienced rehabilitation professionals. Because each person’s needs are unique, treatment is individualized, based on the patient’s physical limitations, abilities, and goals for recovery.

Life Enrichment

There’s always something to do, whether it’s a walk in the garden, music appreciation in the library or that favorite old movie in the entertainment lounge. We offer many activities and outings, as well as scheduled transportation to local medical appointments and shopping. At OakView, residents make their own choices and create their own schedules based upon their interests, routines, and abilities.

Oakview is a fabulous facility if you have a loved one that needs care following a recent hospital stay or due to a change in situation. The nursing and rehab team is knowledgeable, caring, responsive and dedicated to their craft. The facility has top notch equipment and is well maintained. I

My family and I are profoundly grateful for the extraordinary care my husband, Jim, received at Oak View Skilled Nursing. When Jim was admitted on March 17th, following a fall on March 1st that resulted in a brain bleed, he was unable to speak, eat, or even sit up on his own. It was one of the most difficult times our family has ever faced. What happened next at Oak View was nothing short of remarkable. Through the skill, dedication, and compassion of the entire team, Jim made incredible progress—so much so that he was ultimately able to walk out of Oak View May 4th using a walker. That transformation speaks volumes about the level of care provided. A very special thank you to Maria Tang-Itow, Director of Nursing, whose leadership and orchestration of care was truly exceptional. Her commitment to excellence was evident in every aspect of Jim’s treatment. The rehabilitation team, led by Director of Rehabilitation Michelle Clark, worked absolute wonders. I especially want to recognize Chris, one of the physical therapists, whose patience, encouragement, and expertise were instrumental in helping Jim regain his ability to walk. He never gave up on Jim—even when progress felt slow—and that made all the difference. The nursing staff was equally outstanding. Hilda McCrossan is the kind of nurse every patient hopes for—compassionate, attentive, and deeply committed to her patients. She, along with the entire nursing team, treated Jim with dignity and kindness every single day. The CNAs, including Ana Cortes, went above and beyond. They didn’t just care for Jim—they took the time to know him, and even our family. That personal connection meant more to us than words can express. It is clear that the facility’s leadership, under Evelyn Barton, has created a positive, supportive environment where staff genuinely enjoy their work—and that spirit carries through to the level of care patients receive. We feel incredibly blessed that Jim was placed at Oak View during such a critical time in his recovery. The care he received bridged the gap between hospital and memory care in the most meaningful and life-changing way. Thank you, Oak View, for giving my husband—and our family—hope, healing, and compassion when we needed it most.

I feel incredibly fortunate that my loved one was admitted to Oakview for a short-term rehabilitation stay. Every department truly went above and beyond to support us. The admissions team was wonderful to work with, they guided me through every step, answered all my questions, and made what could have been a stressful process feel smooth and reassuring. The nursing staff and CNAs were exceptional, kind, welcoming, knowledgeable, and highly skilled. They provided excellent care while always showing genuine compassion. The OT and PT teams were top-tier; they worked diligently and offered constant encouragement, helping ensure real progress was made. The facility itself is beautiful and impeccably clean. We were also impressed by the quality of the meals and the effort made to accommodate special dietary requests. Residents are engaged throughout the day with activities, and it’s clear they are treated with dignity and like family. The front lobby staff and nursing clerk always greeted us with warm smiles and were eager to help with anything we needed. When it came time for discharge, the social worker went above and beyond to connect us with helpful resources for continued care. We were genuinely sad when our stay came to an end. Oakview is a truly special place with an incredible team. Thank you, Oakview, for everything.
