We reported them to Medicare and PA Agencies. To quote State investigator we are working with, “Oh we’re very familiar with this place and the staff names you’ve provided”.

This was my second time at Maple Ridge Nursing and Rehab Facility. I LOVE THIS PLACE!!! The staff was phenomenal from the Nursing Supervisor to charge nurses and aides. They knew what they were doing and helped me tremendously. Always respectful of me and my wishes. And the Physical Therapists and Occupational Therapists were the kindest, most encouraging and helped me to walk again and get up and down stairs so I could return home. And Jen, the wonderful social worker who included me in my care planning, was sweet and kind to me. I couldn't have done it without all of you. You are all awesome!!! Thank you.
