So happy that my father went to Maple Grove. He improved his physical skills tremendously during his physical therapy rehabilitation. He went from not being able to turn over in a bed while in the hospital to sitting up, getting out of the bed, and using his walker for several hundred feet. The staff is very competent and extremely friendly. Five stars for sure!!!

My mom Helen Prewitt)was there 14 months and the staff was fabulous. They treated my mom with the best care. She loved all the nurses and staff and they loved her. The day she went to the hospital they put her on the vent and the staff came by to see her the next 2 days to say there goodbyes. I will never forget them and the love they showed my mom.

The Masonic Rehabilitation Facility in Shelbyville is nice. My Mom likes it. Therapists are nice. It should rate 4 stars, but we have a big issue with care and communication and that is why I give it a one star. My mother is an nsulin dependent type 2 diabetic. She was released from the hospital rather abruptly to the rehab facility. Somehow the rehab facility did not realize that she was diabetic for the first six days. I believe that the hospital failed to clearly communicate that Mom was insulin dependent. Intake at rehab failed to note that Mom is diabetic and insulin dependent. Mom keeps a list of her medicines with her and did ask about insulin. She did not receive glucose testing, insulin and other standards of care for a diabetic for six days. UPDATE: The Facility reached out with an honest desire to improve by educating staff and improving their end of the hospital to rehab handoff. They were able to catch other cases of patients with incomplete hospital records being released into their care from hospitals. Mom is doing ok.

I couldn't be more thankful and appreciative of the excellent staff that takes care of my Mom! From the Janitorial, CNA's, Nurses, administration and Food Service employees - everyone makes this a clean and loving environment. Very friendly and attentive to residents and family members! Having my Mom at Masonic and so close to my home has been a blessing for my family! Thank you!

When my mom first got to Masonic, it was wonderful. She was in the rehab portion and everyone was great particularly Katie. However when she moved to the personal care section, things got bad. First there was a mouse in the room and they acted like that was normal. Secondly, I feel Sally Garcia was very confrontational with my mother and probably shouldn't be working with seniors. Lastly, they said she couldn't get physical therapy in personal care and I found out later that wasn't true. I am really disappointed because this facility is close to my home and I really wanted my mom to live there.
