Came in not able walk w/walker. After 2.1/2 weeks of excellant therapy & nursing can now walk with walker. Any questions I had about husband's care was immediately addressed by nursing,therapy. dietary.& admissions. First experience in N.H. and was awesome due to the such a caring staff. Definitely recommend Logan Elm N.H

While my loved one was being cared for Rehab in transitional area. He suffered 2 falls unwitnessed. Both times I was notified of the falls , but they said there were no injuries. Upon my checking him, I noted a large bruise noted on the RT side of his back and he had a skin tears both times on his elbows that had bled on his sheet. I went to the desk to get a bandaid and wasn’t even asked why he needed it. I did inquire about him being assessed for injuries and was told yes, but nobody had a reason for the bruising. His family had to make a call to his vascular surgeon to inform him of the unwitnessed falls and both times he was found on the floor. He insisted that he go to the nearest ER got scans from head to toe. I had to take him myself. I went in one day while he was in therapy I went back to his room and the lid to his meal was off his food and had been laid on his bed. Another time I went into s e him and he was sitting up on the side of his bed , IN THE DARK , trying to eat his food. He had accidentally dumped his lemonade d/t not being able to see it. I went o nurses desk and was told housekeeping was gone for the day and that she had no access to a mop , but she said she would wipe it up. She put the towels on the spill and jumped back and said that it as urine. I told her most definitely did not urinate on the floor. The nurse was not apologetic at all. The STNA was very apologetic and kind. The nurse treated my loved with an uncaring care and no respect at all . They complained that his family brought in fast food and sweet tea no wonder he was sick. They were told that his family had brought it high protein oatmeal honey an fruit and nuts for it No one made it for him as he continued loosing weight. The nurses said that he was refusing his stool pills , but all of them were mated off as being given to him. Our experience was a very bad one and he will never be there again. I never did witness any resolution to any of my complaints.

Family member was discharged from hospital for rehab to Logan Elm. Arrived in the afternoon, overload of people and questions when they arrived, which is absolutely normal and to be expected. Nurses asking questions, dietary asking about likes and dislikes while taking notes to ensure meals were something they would like, which was nice to see and hear. Then it all went down hill. Call light response time was average of atleast 50mins. 50MINS for a person to use the restroom. That was the only time they had used the call light was for assistance to restroom. Everything else they could do! I will say the best part of the short stay was the aides!! They were kind and always respectful and I NEVER seen them just sitting around. The first 50min call light, I was visiting and went out in the hall after 35mins. The nurse was sitting at the nurses station scrolling on her phone as his call light was actively beeping right next to her on the machine, when I asked if he could get assistance she stated "I'm sure there busy, I'll find someone" and got up to go find someone. This happened 2 times when I was visiting that the NURSE was just sitting when call lights were going off! Then there was the therapy! When we had finally had enough (condescending attitudes from nurses and management) when we said we were leaving, the therapist said you need to walk with a shoe on and cane. Perfectly fine, I brought a shoe and cane. The heavy set, grey hair lady with therapy (dont know names because EVERY person in the facility has their name badges turned around..very convenient) says "Well let's see it" we asked what you want us to do and she said "Well walk, let's see it,go!" I went to set the cane over by them and she grabs it, lifts it up and sets it back down on the other side of the bed while saying "NOPE, he wants it, he gets it". There could be so much more listed in the short 4 days stay, but ultimately we ADA, and left. That was enough. I had high hopes for this stay but it turned into complete failure with astonishing acts of "care" and morals from almost every person in the facility on the transition side! They work very short staffed and apparently weekends are the worst! That of course being im sure due to management/scheduling not working weekends. They're not there and they don't care! The majority of "lack of care provided" comes from not having enough aides on the floor to provide the care for the amount of people in there! Mary in scheduling, do better! I hope you never get to a point where you need a facility to take care of you! I hope you never have to wait 50mins to have a bowel movement, to hold your urine for 50mins when you only have 1 kidney functioning and it barely functions! The nurses and admin, I hope you never have a person talk down to you that is with a facilty that is supposed to be promoting your health and healing, i hope you never are down on your luck recovering just wanting some help and know that evertime someone ealks through that door to help, they will be "griping " at you about something. I was raised taking care of people and continue through my adulthood! You NEVER treat a person like that! You always gove a person DIGNITY and RESPECT no matter what they are dealing with! Big and small to you doesn't mean what a person is going through is not valid. People dont go to rehabilitation or nursing homes when they are in the best days of there life! They are there when they are in need! We were told the reviews and complaints are reviewed by a admin at the facility. Then when asked what then happens, it was basically they read them and then tell the person there was a complaint about them and then NOTHING! They just go on to the next!

Our past experience with care provided for family & friends was positive. Our most recent experience as guest of resident activities was outstanding! The staff loves each resident as they become family and are willing to devote time and energy to ensure pleasant gatherings and outings. We are happy to participate in activities and donate to provide opportunities for everyone. Keep up the great work. John 13:34 Love one another, As I have loved you.
