The Loft Rehabilitation & Nursing is a senior living community in Eureka, Illinois offering Skilled Nursing / Long Term Care. Amenities include Restaurant style Dining Room, Gathering / Activity Spaces and Housekeeping. Families rate it 3.9 out of 5 across 38 reviews.
The Loft Rehabilitation & Nursing is a 92-bed skilled nursing and long-term care facility in Eureka, Illinois, serving residents on Medicaid and private pay.
Family experiences at The Loft divide sharply. Several families describe consistently kind and attentive staff, noting that employees work with grace despite staffing pressures, that residents have private rooms, and that the facility itself is clean and well-maintained. One family wrote, "every day she tells us how nice all of the CNA's & Nurses are." However, other families report serious concerns: instances of residents left in soiled conditions for extended periods, inadequate monitoring and hygiene care, missed or delayed transportation to appointments, and a vaccine administration dispute with a family who had explicitly requested their mother not receive it. These accounts describe neglect severe enough that one family reported bedsores and is pursuing legal action.
With a 3.9/5 Google rating across 38 reviews, The Loft shows polarized feedback. Five-star reviews consistently praise staff warmth and facility cleanliness; one-star reviews cite safety, hygiene, and communication gaps. Prospective families should conduct thorough visits and ask detailed questions about supervision protocols and medication consent procedures.

This place has really disappointed me. I have a loved one there that has been left soaked in pee while multiple nurses are standing around chatting. Nurses are also not educated on how to transfer patients from bed to wheel chair etc… Edit to add: My love one has now passed away. She had a stage 4/5 bedsore from pure neglect of this place . These people just care about getting paid and not about the health of the residents. Dementia was going to take her one way or another but this place caused her pain and quickened the process from neglect. I’m absolutely mortified. I DO NOT RECOMMEND THIS PLACE.

The Loft N.H. Eureka IL location was very disapointing in all areas while my mother was there. If you have a love one you just need to drop off The Loft N.H. in Eureka IL is the place. This facility needs to be inspected by the state of IL due to lack of care and attentive to the people who are placed at this location, not by choice. Do your research!! We had no choice since my mother was transfered from another Loft in IL . I was a POA for healtcare for my mother. I requested documents concerning my mothers care while she was at the The Loft in Eureka they claimed was done daily during her stay, it was never given to me. From experience if you do drop your love one's off . Take the time to do unexpected visits. Again I hope the state of IL for healthcare will investigate THE LOFT N.H. of EUREKA.

Yes, if talking with someone from the corporate level will improve the care for the patients that are still there.
