My grandma has been here for over a year and the quality has greatly declined. She's been in both skilled care and assisted living at various times. Under staffing has caused significant issues. She's left in bed until 3 pm many days even though she's supposed to be moved to her recliner and have her clothes changed at breakfast, I witnessed this today, and the aide apologized to me and said he's the only one most days and he's doing his best. Many times she calls to get assistance to go to the restroom and her call light is left unanswered for an hour or longer. Her denture broke 2 weeks ago so she can't chew food, we asked daily for her to be put on a soft diet until they're repaired and as of today, the nurse finally got her added to a soft food diet list. They simply don't have enough staffing to appropriately care for patients, especially those that can't get around without assistance.

The positive difference Liberty Station Health Campus has made for our 94 yr old dad! He is thriving in long term care and loves the food, caring staff, activities and atmosphere. He is the happiest we’ve seen him and he tells us he is happy. The therapy department (PT/OT/speech) is top notch. We transferred him last month from another facility that did not meet expectations or his needs. I highly recommend LSHC for rehab

As a healthcare partner- this facility is wonderful to work with. The staff are always very responsive and willing to help. The facility is new and has a luxurious feel inside. I feel confident in recommending my clients to their community.

Looks can be deceiving, it’s sad.<br>If your loved one Needs therapy or any short term care and might have some memory issue I suggest you not go here. They don’t have the staff or the knowledge to treat the elderly with dementia or any health issues. Their treatment is to sedate them and then try to get them to do therapy while they are still drowsy and then says oh they can’t do it they will not stand so they do bare minimum for therapy. Try to give wrong meds 10 times I caught, was waiting on bloodwork results that they said Is being drawled weekly come to find out it was done one time in a four week period of time and then one more time before the day of discharge cause they had too, on top of 30 plus falls in the 4 weeks time, 3 alone just in 1 day. Was Diagnosed with a UTI took a week to get antibiotics started after we stayed on top of them asking for the results. I could go on and on. Bottom line if your love one Needs very minimal care and has no memory issues and can care for them selves, yeah maybe. But if your loved one needs more this is not the place for them. After discharge we was told a nurse would be out the next day and OT and PT would be out, let’s just say they will be out 9 days after we been home so here we are 10 days without OT or PT on top of we saw family dr same day we left and let’s just say they was not in contact with the dr with my love one being stage 4 kidney failure which I told them a good 10 times and family dr said why did they do all these meds these are not good for the kidneys as it is. IF YOU GO THERE JUST BE CAREFUL
