I cannot begin to express my gratitude for the exceptional care that Liberty Commons Nursing and Rehabilitation Center provided to my mom after her hip replacement surgery. She was there for short-term rehabilitation, and the support she received made all the difference in her recovery. First and foremost, I want to extend my heartfelt thanks to Melanie, the case worker. Her organization and attentiveness were remarkable. She answered every question my mom and I had and coordinated with various services to ensure a smooth transition from the facility to home. I also want to acknowledge the incredible nurses, Jennifer and Brittany, who consistently went above and beyond to check in on my mom, even when they were not assigned to her side. Their dedication truly made her feel cared for. A special shout-out goes to the Physical Therapy and Occupational Therapy Department, as well as Ms. Lydell, the Activities Coordinator, who organized some fantastic events that kept my mom engaged and uplifted during her stay. I must also recognize the hardworking CNAs, including Jalaysia, Diamond, Ashley 1 and 2, and all the others who provided compassionate care. In my opinion, they have one of the toughest jobs, and their efforts did not go unnoticed. Lastly, I want to give a huge THANK YOU to Angela. She consistently checked in on my mom every day she worked, and even on days she wasn’t assigned to that hall, she made it a point to ensure my mom was comfortable and had everything she needed. I believe she deserves to be recognized as a lead CNA, as her dedication comes straight from the heart. Thanks again to everyone at Liberty Commons who made my mother's stay comfortable and supportive. Because of your hard work and commitment, she is now back home and doing great! C.
