My Mom received excellent care at Liberty Commons. The nurses, CNAs, and PTs she had were excellent, the facility was clean, and while you hope your parent never needs a place like this, it’s good to feel like they’re well cared for if they do.

My dad was a patient there for rehab 3 weeks last year. He was in rough shape when he arrived and in even worse shape when he left. He was left soiled for over 12 hours many times, nurses made excuses for the cnas not doing their jobs, was marked as refusal for therapy when he wasn't refusing, was told he couldn't leave by ambulance at discharge (a lie). When he arrived he could walk with assistance when my mother took him back to the hospital, it took 3 staff members to get him into the truck. He couldn't even sit up by himself. When they got back to the hospital the doctor said they didn't know if they were going to be able to save him. Do NOT take your loved ones here, it is a horrible place and the employees do not care about their patients.

I would give less stars if I could. My 58-year-old mother is a disabled patient at Liberty Commons Rehabilitation Center, and the care she has received has been unacceptable and dangerous. She has a complex medical history, including cancer, a stroke, diabetes, and a recent hospitalization where she was on life support for three weeks due to sepsis from the flu and RSV. She was sent here for rehabilitation and physical therapy. Since being in their care, she has fallen four times. She cannot walk or transfer to her wheelchair independently, yet she has repeatedly been left without assistance. When she presses the call button, it often takes up to an hour for staff to respond. Despite being diabetic, she is not provided diabetic meals. Her medications are not administered as prescribed. She has had accidents and developed sores because staff failed to help her clean herself or change in a timely manner. This morning, she developed a severe migraine around 6:30 a.m. and ended up vomiting on herself. She has had to call my sister, brothers, and myself, for help because no one responded to her call light. She was not given even a Tylenol until 11:00 a.m. She is now being transferred back to hospital due to a preventable fall, that she has sustained a head injury from. The staff is consistently rude and dismissive. They have told her to hang up the phone with family before speaking to her. There is a clear lack of compassion, urgency, and basic standards of care. No family should have to worry that their loved one is unsafe in a rehabilitation facility. I would strongly caution anyone considering Liberty Commons to look elsewhere.

Rehab personal was great, some of the staff were good. The food for my family member was terrible. One time I was there during lunch and my family member couldn't eat it and they just took it and didn't bring them anything else. Sad these are people that depend upon them with no other choice. I personally would not recommend. Plus there are no single rooms even though they say there are.

My grandma spent the last month of her life in this facility. I have to say I was extremely pleased with the service. I was there daily and the care was top notch. She was always clean and in clean clothes. They went above and beyond to care for her. They were always pleasant and kind to me always answered any questions I had and willing to help anyway they could. They were with my grandma at the very end to and made sure she was peaceful and comfortable. I know there’s some bad reviews on here but from my experience with my grandma I would 100% recommend this facility. Shoutout to all the staff on the 400 hall that took care of my grandma. They were truly the best.
