I can't say enough about the care provided at Legacy Nursing Home in Pleasant Grove, AL. My nephew is 49 years old. He had an auto accident when he was 16 that left him paralyzed from the chest down. He cannot sit in a regular chair, nor stand or walk. He requires help to get dressed and undressed and a hoya lift to get to and from his bed and wheelchair. He has been paralyzed for 70% of his life. He requires more than average care to function than many people in a nursing home. I cared for him for the last 8 years after his mother died suddenly. His father died 25 years ago. He has no immediate family and has been at Legacy for the last 9 months. The entire staff of Nurse3s, CNAs, Aides, and Administrative all under the leadership of David Thornton make every effort to provide the special care he needs. I appreciate so much, people like Barbara Carter and Crystal Barber Charge Nurses, BreAnna Smith, Quintella Clopton and Irene Muetembei, Nurse Practitioners, David Jackson, Wound Nurse, all have his health and wellbeing first and foremost and work as a team to communicate. As well as Jessica Felder, Social Service, Kimberly Brickell Financial Director and Donna McCaain, Dietary Manager. This is not to say that everything is always smooth sailing 100% of the time. Life doesn't work that way. I have learned that caring for 150+ people with various needs, deadlines, priorities and feeding 3 meals a day is a daunting task. Legacy measures up! I also appreciate Mr. Thonrton's open door policy to meet with residents and family members. Sincerly, Robert E. McArdle

Legacy has a large outdoor area that residents can enjoy. Our friend who was there several years appreciated that area daily. She has transferred to a facility in another state to be near family and friends. Legacy continues to support transferring her information to her new facility. Kim Brickell, the Legacy bookkeeper has been particularly helpful and responsive. Jessica Felder who was the social worker at the time of the transfer was also very helpful.

My Father spent approximately 2 weeks in this facility. To say we were truly disappointed and appalled at the care he received is an understatement. Therapy services were fantastic and did not disappoint. They were the sole reason we selected this facility. However the nursing care was terrible. As a nursing professional (RN since 1986), I could not believe a patient asking for a urinal, bedpan, bedside commode would consistently be told by the staff to “just go in the bed and we’ll clean you up later”. I could not believe a patient would be in his room at 1:30 PM lying in a soaked bed with his urinal lying in the floor and urine all over the floor; door closed; call light on; staff on the front patio smoking; no breakfast and lunch tray still in cart. My father scoffed his lunch down because he was starving. Had I not popped up that day I guess he would not have received any meals. The staff was quick to lie “we gave him Ensure”. My father denied having Ensure and as dirty and unkept as his room was, the Ensure container would have been in the trash at best. No evidence of Ensure was found. This was not an isolated incident and proved to be a recurring theme during my Father’s stay. I could not believe, that within one week of admission, the social services director and financial counselor approached my 81 year old mother telling her “your husband won’t be ready to go home after his twenty days so we need to get him signed up for Medicaid. We need the deed to your home and six months of your bank statements”. When my mother told them she wanted to talk to me about it but I was out of town. She was told, “We need the information by Thursday” (It was Tuesday afternoon). When my mother told them her daughter was a RN who works in healthcare compliance they left his room and had not returned when he discharged to the acute care setting. Additionally, during his stay his condition worsened and surgical site worsened. I removed my father from this facility never to return. As a seasoned medical and compliance professional, I do not recommend this facility.

All I’m gonna say is If you love your loved one and you care about them don’t send them there cause they will not be treated right my dad wasn’t there a whole month and He is back in the hospital with pneumonia, UTI, and dehydrated. This place is horrible!!!!!

My family recently had a family member stay there about three weeks. Things seemed to be okay at first. Most of the staff were friendly to many of the family members. They stayed informing the family members' children about how they were doing in the room. The therapists (occupational, physical, and speech) were great with our family member, and was able to tell us what they were doing good and not so good in. On the family member's third week there, they got sent out to a hospital in the area due to them going into respiratory distress. However, the children of the family member found out from the ER doctor and nurses some information from the EMT's that contradicts some of the what the staff at the facility said. You can image how the children and family members are feeling about it. Afterwards, one family member went to the facility to pick up the belongings, and once one of the children got their parent's belongings and went through it, that child realized that are items that are missing and shouldn't been used up in a three week stay. We are disappointed at the facility, but we do pray and wish them well. We hope the staff will step up and do better across the board. If you work there and take care of the patients, you should take care of them like your closest family and friends. Love of them, take care of them If you can find a place that will love and take care of your family member as they are your family, please that them there.
