Do not send your loved ones here if you care about them! My grandma was sent here straight from being in the hospital for 2 weeks. She was on the rehabilitation side to get her strength back up, then she was going to come home. My grandmother didn’t even make it 24hrs in this facility before being rushed by ambulance back to the hospital from this facilities negligence! The staff overdosed her on a muscle relaxer she wasn’t even taking anymore. Then after she was unresponsive for over an hour the staff called my mother?! My mom was in tears saying why’re you calling me? Call 911 now, then when we arrived at the ER the ambulance driver was surprised to see my mother and I so quickly. I said that’s because the facility called us before you, the driver also informed me that she asked the staff how long my grandmother was unresponsive, they said at 5:30 she was found unresponsive. The ambulance driver said it’s 6:30 why did it take you an hour to call 911? The driver said the CNA/staff member would not answer the question. Because there is no excuse why it took an hour to call emergency services. Thankfully my grandmother recovered and is in a different rehabilitation center. When my mother and I had to go into the facility to get my grandmothers belongings it was so sad to see the condition of that place. People just left in wheelchairs all over not being cared for, the smell was horrible, we also hears someone screaming I wanna go home over and over. No one was able to tell us where my grandmothers room was, we had to go looking in all the rooms to find her belongings. They were just left out, for anyone to take. I know no facility can be perfect but this place was unacceptable.

My 87yr old Mother was at Lake Park for rehab after a major surgery for 3 wk. She was in a wheelchair and a fall risk on arrival. Prior to surgery she was living independently in her home. She progressed to walking 375 ft with a walker. She is home now and moving around her home without the walker and performing "light housekeeping." So grateful to the staff. Many very kind and capable health care workers there. I would recommend trying to avoid admitting on a weekend. Some of the administrative staff are off, which is expected and the admitting process and normal "orientation" is delayed. Sometimes that is unavoidable, which is what happened in our case. Be patient and it will smooth out. They are probably short staffed as most places are in the health care industry. The rehab department is amazing. Thank you staff!

My uncle checked into Lake Park for hospice care in December of 2025 and passed away about 7 weeks later. During that time, he was well looked after, with kindness and compassion. The staff are lovely. They have high standards and caring attitudes. I would recommend Lake Park for anyone in my uncle's situation. Tip for anyone: If you transfer from a hospital to a nursing home on the weekend, make sure all the patient's details, including prescriptions, are properly communicated.

Firstly I'd like to praise the occupational and physical therapy group. These people really care about their patients. (Jay, Cassie, Michael, Christina, Mike and others whose names elude me,,, I'm so grateful) The nursing staff were always available to answer all my questions and checked in on me above and beyond my expectations. The folks who brought me my medications, (shout out to Aman), were friendly and willing to listen to my many questions. Even the people who brought my food were caring and always willing to get me answers if they didn'tknow them, (shout out to Doh, probably spelled wrong). Special thanks to Meghan for extra help every step of my stay here. I didn't come here for a vacation. I came for rehab and an education on how I can live a more healthy life and to help me on the road to a full recovery. I got that and more. I'll miss these caring and competent people. 5 stars , I'm lucky that I chose Lake Park.

My dad did a 3 week stay at Lake Park Nursing and Rehabilitation Center after he had a stroke. The care he received was wonderful. The staff were caring, engaging, and helpful. The social worker, Meghan, was incredibly kind, informative, and went above and beyond to make sure we understood processes and how to get in touch with her. Most everyone I encountered there was friendly and willing to assist when we had questions or needs. Very pleased.
