Immanuel Pathways - Southwest Iowa

Immanuel Pathways of Southwest Iowa is a PACE® (Program of All-Inclusive Care for the Elderly) program in Council Bluffs, Iowa, serving adults ages 55 and older who need nursing home-level care but wish to remain living independently at home. Since 1887, Immanuel has provided faith-based senior care and services across the region.

The program provides comprehensive, coordinated, and customized healthcare and social services to participants, with a dedicated Care Team coordinating all aspects of care. Services include primary and specialty medical care such as audiology, dentistry, optometry, and podiatry; home care; prescriptions; specialty care; respite care; transportation; adult day services with meals and nutritional counseling; social work and personal care; and physical, occupational, and recreational therapies. Hospital and nursing home care are provided when medically necessary.

PACE® covers all Medicare Parts A, B, and D benefits as well as all Medicaid-covered benefits and additional services deemed medically necessary to maintain or improve health and well-being. Medicaid-eligible individuals pay no premium for Immanuel Pathways services. Most participants are dually eligible for both Medicare and Medicaid.

Immanuel Pathways serves residents in Pottawattamie, Harrison, and Mills Counties in southwest Iowa, and is recognized as a Level 2 Age-Friendly Committed to Care Excellence program by the Institute for Healthcare Improvement, reflecting its dedication to delivering personalized care for older adults living independently in the community.
