Utterly disappointed in the unsafe discharge that happened today with my friend. She has no family and is unable to walk. She was dropped off to an apartment with no care plan in place. No phone calls were returned to her support people, home health agency or her case manager. Totally unsafe. This has been reported.

I had a very good visit with several residents and several staff were very helpful in directing me to each residents room and answering my questions. The facility appeared very clean and had a welcoming atmosphere.
