I agree with the previous person who wrote all about the facility and will add a little of my own. The photos and video on Holly Heights’ website are easy to see just for show and does not reflect the level of care my mother received during her stay there. My Mother reached out to me upon arrival to communicate her distress with the state of the facility. After she called me I started to takes chronological notes and the overall experience revealed a troubling lack of attentiveness, accountability, and professionalism throughout the facility just like what I had read here on a previous review. My mother became a resident at Holly Heights after fighting kidney disease and heart troubles with the idea of gaining strength and going home. Staffing was less than minimum and the quality of care for vulnerable residents is something like 1 nurse to 20 patients. During my mother’s stay, she received very little consistent care or attention. Serious concerns included: * Food being served that was disgusting to look at and eat. * Food portions and diet not correct for her condition. * Upon arrival was made to sleep in a broken chair. * Ongoing dehydration concerns. * Lack of basic supplies needed for her care. * Medication administration such as skipping pain meds. * Staff repeatedly telling her to “call if she needed anything,” then not responding for multiple hours. * Water left unchanged. * Food trays sitting for extended periods. * My mother positioned improperly later on a broken bed. * Food residue left on her face and hands. * General neglect requiring immediate intervention. We filed an official grievance with the the State of CO and the facility. The cleanliness of the facility was also concerning. Floors were not cleaned during the 6 day stay. For anyone considering this facility for a loved one, you may choose this place due to your lack of funding however I just like others who are commenting strongly encourage you to remain actively involved, ask questions often, and advocate continuously for their care.

My grandmother received very poor care here before almost passing away due to their negligence. She is now in a new facility. I would not recommend this facility for your loved one.

I had my mother transferred to Holly Heights in Jan from the Hospital. Under Innovage I was given two choices and the reviews looked pretty good. After her being there a few days it became clear that she wasn't thriving there. When a patient doesn't eat you don't just place food on the side table and leave. You make sure they eat. Her room mate (who was memory impaired) was wearing her clothes and she had moms coat on her chair. I complained and the nurse didn't seem to concerned. Even when transferring mom out of that center her room mate had taken her phone to her side of the room. Mom wasn't getting changed and cleaned. I walked into the room several days after mom had been there and the whole room smelled of BM. When I asked, they then changed her. When we transferred mom to another care center she had dried BM on her bottom. The nurses were nice, but hard to communicate with because of language barriers. When I would call the nurses station mom's room mate would often answer the phone! She was out an about, wearing my mother's clothes and answering the staff phones. I dreaded going to visit mom at that enter. There was always something I didn't like going on. I would never recommend anyone being placed at that Holly Heights.

I am writing this review to inform other families about this facility before it is too late. My mother lived here for over two years and initially received acceptable care until they changed ownership. After a room and care team change in November 2025 that I was not given any choice about, I observed a clear and serious decline in both her condition and the care she received. During this time, there were repeated issues. The most serious incident occurred when she was found in critical condition—her oxygen was off and her air mattress had deflated, causing respiratory distress. A hospice nurse arrived just in time for a scheduled visit and intervened; if not, she would have died. On another occasion, I raised urgent concerns while visiting about a possible infection. The physician there refused to come and evaluate her in person or provide treatment that I asked for at that time. Her condition continued to worsen, and I made the decision to remove her from the facility a few days later. She was then hospitalized and diagnosed with fluid overload and pneumonia and she has continued to decline. After her hospitalization, I moved her to another facility. I also filed complaints with both the Ombudsman and the state which took over 4 months to get a response and there was no action taken. Public records also reflect a large quantity of other occurrences filed against this facility including sexual abuse. Based on my experience, I do not recommend this facility. I would strongly advise families not to put their loved ones here and this facility should not be able to continue to operate as it currently does.

I came to visit Dad and I was gonna put a rug down for him, but the floor was dirty so I asked the lady at the front desk if I could have a broom and a scoop to sweep it up and she said I’ll get maintenance.<br>within five minutes maintenance was there sweeping the floor for me.
