Heritage Gardens is a clean, lovely facility in Loma Linda that was a great place for my wife to go for physical rehab. There are Physical Therapy, Wound Care, RN Services, Social Activities and more. I would like to recognize our Case Manager Tanya she went above and beyond helping us understand the way things work and was very patient with us.

My mother in law was in this place for 4 days before we removed her. HORRIBLE PLACE. If you want your loved one to die. Send them here. One day she was talking and eating the food we brought to the faculty because she didn't want their food. Then she wasn't taking or eating. No one could tell us what happened. Everything that was supposed to be routine for her care we had to request for it to be done. Once we started asking questions, about the changes in her health they had the nerve to say we were stressing out their staff. WOW. Sorry we were just asking them to do their jobs.

On the date of discharge, my mother was released from this rehabilitation facility despite being physically unable to walk safely or manage stairs. She lacked sufficient leg strength to climb the ramp of the transport van, yet the discharge proceeded. During transport, she attempted to use her walker and fell from the ramp while the driver laughed instead of assisting her. Upon arriving home, she was unable to climb the stairs as she had previously warned the facility’s administrator multiple times. We were forced to contact the local fire department to help carry her safely into the home. My mother had informed the administrator on November 6, 2025, that returning home would be dangerous because she had only practiced stair climbing three times during her stay—all within the last eight days. Despite these clear safety concerns, staff told her there was “nothing medically wrong” with her and that she “needed to go.” This discharge violated basic patient safety and dignity standards. The facility’s actions demonstrated gross negligence, lack of compassion, and a failure to ensure a safe transition of care.

When I leave a review, I try hard to leave emotion out of the narrative but if you/loved-one need(s) 24/7 Skilled Nursing Care, this is not the place to go (at least not in my experience). Patients who are in need of regular attention, Elderly, diabetic, incontinent, recovering from surgery, may want to look elsewhere for care. Like most such facilities, The bad will be what you remember when witnessing negligence, complacency and slow unprofessional care, particularly with elderly loved ones. I try to remember that several patients need care/help and so things don't always get done quickly. But when a recovering bedridden patient lays in their own waste for several hours contaminating their surgical wound and medical needs persistently ignored, it's hard not to feel contempt. I found it incomprehensible that the staff who work with patients in this facility had absolutely no knowledge of wound care and how to care for older, limited strength/endurance patients. No bedside commodes for those, who are bedridden. My mother was discharged against doctor orders with Absolutely no plan for home care visits, no follow up, and a surgical wound that was so corrupted that she was forced in to another hospitalization. I feel that there are staff there that care, but overall they are either not trained for how to care for certain cases, or they are not Skilled Nurses. There are some staff who do make the effort to help patients, but in the end, that cannot make up for the overall shortage of professional doctoring and caregiving. I wanted to "love" this place, after all, my mother was sent here by the hospital with no option for a closer and better suited place for her benefit. REMEMBER, I learned the hard way that you do have a choice when a loved one or yourself is placed far from home, or in a facility that may not be best for all concerned. And if you have a choice, look to a better rated, better caring overall outfit.

My mom was sent here after multiple falls at home for rehab. Her stay here was wonderful. They worked on getting her stronger as well as keeping her active in activities like bingo, which she enjoyed and we did too! The staff was attentive and kind. They know all the patients and take extra care to know what they like or don’t like. It gave us a lot of peace knowing she was in good hands. We are grateful she was ready to come home today .
