About Fulton Commons Care Center

Fulton Commons Care Center is a 280-bed skilled nursing and long-term care facility in East Meadow, New York. It serves residents requiring nursing care and rehabilitation services.

Family experiences at the facility are markedly mixed. Some families describe exceptional individual staff members, caregivers and nurses who show genuine attentiveness and compassion, while others highlight systemic gaps. Concerns raised include inconsistent room preparation and hygiene practices upon admission, difficulty reaching residents by phone, and inconsistent continuity of care across shifts and units, particularly problematic for residents with dementia. One family noted that while some shifts have cohesive, family-like teams of aides, staffing gaps appear to create uneven care experiences.

The facility holds a 3.4 out of 5 rating across 146 Google reviews, reflecting this variability. Prospective families should expect to evaluate specific units and shifts carefully, as quality of care appears closely tied to individual staff members rather than uniform policies or systems.

The Fulton Commons in East Meadow is a CREDIT to Patient-Care EVERYWHERE. Today, I depart, and so it is with a Heavy Heart that I leave the likes of Genuine Heroes who preside over DAILY MIRACLES!!! I am honest-as-the-day-is-LONG!!! I say what I Mean...and Mean what I say. Ms. Laura DiBlasi tends to her flock with a meticulous certainty that highlights the ever-present Mojo/Magic of this Heralded Healthcare Facility!!! The Aides are the crux of the Care. I had my Daytime Marie and my Evening Marie. All had that motherly spirit that one would swear leaves this world with one's Parents. Then, the RN's, who preside over each shift like watchful Hens (or Roosters, for that matter), making sure Meds AND Patients alike are collated with True Medical Precision. Margarita led the group, along with Neja, the RN who handles the proper Front Desk Administration so well. I missed her when she was not well for a few days.. So many stars, it'like Camelot...or a Hollywood Red Carpet (Limelight...IS MY SOFT DRINK!!!). Then Nurse Belgrade., Maxine in the overnite, and a veritable Army of ultra-Professional Nursing Stars!!! Then there is Physical Therapy. The Radiance of Alexis, The No B.S, Jack, the ever humorous CJ and Evan and Dianne and smiley Grace. Not to forget Ultra Mega Cool Courtney, paired with the Graceful Afifa, Robert, the Silver Fox, and so many such other Majorly Cool Physical Therapists. All watched over by the Omni-Present Kevin.. The Housekeeping guys were ALL Cool Cats, who I swear I would see on TMZ, my-one-guilty-pleasure-Gossip-TV Show!!! Stars, ALL STARS!!! This well-oiled machine functions with Swiss Precision. It has been my honor to stand in the light of such Constellations of the Cosmos!!! My miracle came to pass here, also. I came here a bed-bound, no-hope mess. Not 2 months later I not only STAND, I feel I could trip-the-light -fantastic with the Best of 'em!!! I beseech thee, if you are among the sick and broken...COME HERE to watch your very own Miracle come to pass...right in front of your eyes. I saw these True Miracles happen DAILY. I just know God rents a Suite here...You can Literally Feel it. Come and see!!! Donato Quinto Fulton Commons Patient Extraordinaire Departing Today. P.S. Thank you all for me getting my swagger back!!! And Arnelle the ultra-sensitive P/T man!!!...and Social worker Kiawna who holds the whole chain of Healthcare Together!!!

⭐☆☆☆☆ 1 Star I live out of state, so being able to reach my loved one by phone is extremely important. Unfortunately, my experience with this facility has been very disappointing. I have called multiple times trying to speak with a resident because I am concerned about his well being, and each time the person answering the phone transfers me before I can even finish explaining why I’m calling. There is no “Please hold while I connect you,” and no confirmation that anyone has actually answered on the other end. Instead, I repeatedly end up in voicemail. On one occasion, I asked where I was being transferred and was told it was a personal phone (by his bed) which concerns me further that he’s not answering. I then requested to be transferred to the nursing station, only to be sent to another voicemail. The irony is that while you’re on hold, you hear messages about excellence, quality care, and outstanding customer service. Sadly, that has not been my experience at all. Basic phone etiquette and communication seem to be lacking. As a family member who cannot simply drive over and check on a loved one, this is incredibly stressful and concerning. If reaching a resident is this difficult, it makes you wonder how well the facility communicates in more important situations. I sincerely hope management takes these concerns seriously and improves their communication practices. UPDATE (6/8/2026): Today marks the third day that I have tried to contact my family member, and once again I ended up in voicemail. I kept calling back, and eventually they just stopped answering the phone altogether. My family member has only been there for a week, and during that time there has been only one very kind staff member who actually put him on the phone. I wish I had asked for her name because she deserves recognition. If I tried to make this up, I couldn’t make it this bad!

My mother was discharged from NYU hospital on 4/11/2026 Large mass was found in abdominal area Which is cancerous and doctors recommendations were for her to go on community hospice, she need 24 hour nursing care when we arrived at facility, all her items was in boxes her bed was not made then bed was made without disinfecting the bed .. I don’t understand why they will treat my mother this way knowing that she was diagnosed with terminally illness and only has a few months to live.and This is the way she gets treated when the facility was aware of her discharge so I don’t know why her room wasn’t ready for her.. No one came in to do the intake assessment for about 45 minutes after she arrived. The floor to her room was filthy dusty Maintenance was called in all he did was come in and sweep with the mop The bathroom to our room was filthy. No remote for her television, unable to turn television on. I waited about 10 minutes for Nurse to come in to do an assessment and change her clothing to something comfortable iI change her diaper change her into comfortable clothing but no one came in. I cleaning her room myself wiping everything down changing her. administrators were very rude, dismissive, and cut off during every interaction. Whenever I tried to ask questions about care, policies, or my loved one’s well-being, I was interrupted or spoken to in a condescending tone. It felt like they were more concerned about ending the conversation than actually addressing concerns. There was no warmth, no empathy, and no professionalism — just a cold, rushed attitude.Communication was also very poor. Calls were not returned in a timely manner, and when I did get someone on the phone, I felt like I was inconveniencing them. Important information was either delayed or not clearly explained. When you are trusting a facility with the care of someone vulnerable, transparency and compassion should be a priority — and unfortunately, that was completely lacking here. I also noticed a general sense of disorganization. Staff seemed overwhelmed, and it often felt like there was little coordination between departments. Simple requests took far longer than they should have. Instead of feeling reassured, I left interactions feeling stressed and unheard. What was most concerning was the attitude from leadership. The administrators set the tone for the entire facility, and if they are dismissive and unapproachable, it trickles down. Families deserve respect, patience, and clear communication — not to be cut off or made to feel like a burden.I would strongly encourage anyone considering this facility to ask very direct questions about communication policies and spend extra time observing how administration interacts with families. My experience was frustrating and disappointing..

Kia Blackstock is truly an exceptional nurse. She has been caring for my mom (very difficult patient Talat) who has dementia, and the level of attentiveness, compassion, and respect she shows every single day does not go unnoticed. She is incredibly patient and genuinely kind, always making sure my mom feels safe, comfortable, and cared for. What stands out even more is her reliability and dedication—she is the only nurse who consistently calls me back when she sees I’ve reached out, even if she initially misses the call. That level of thoughtfulness means so much, especially when caring for a loved one with complex needs. As a nurse myself, I understand the demands of this profession, and I can confidently say that Kia goes above and beyond. She sets a standard for what nursing care should look like. My husband and I are beyond grateful for her.

This place has “NO CONTINUITY OF CARE”!!! Continuity of care is essential for anyone with Alzheimer’s or any kind of dementia. There are many excellent but underpaid and overworked aids here. We were fortunate to have good aids on 4 East in the morning and an entire group of aids in the evening who work and act like a family and treat the patients as one too. However, The Grand (owners) felt it necessary to remove the entire morning floor and replace with all new. We got the worst aid in the building and my wife’s health has plummeted even after I’ve asked many times to remove that aid and they didn’t. Now the Grand, who own many nursing homes in NY are doing the same with the entire evening shift and nurses! It makes no sense and no one is happy with the change, but they don’t care. While my wife’s now has severe cuts on her legs and stomach from the morning aid, there have been several deaths on the floor (not sure if it’s form the change or just coincidence all of a sudden)! The Grand doesn’t seem to care about the residents and the families, nor the staff. I now hear The Grand is selling the facility; wish I knew who it was to contact them. There is some incredible staff here but extremely poor management! I gave three stars because of the good aids and nurses we do have
