Forest Park Nursing and Rehabilitation Center is a skilled nursing and rehabilitation community that supports both short-term rehab and long-term care residents. The team focuses on helping patients reach their recovery goals while creating an environment of warmth and caring during a stay.

Rooms are spacious and private, equipped with TVs and phones, and residents receive 24-hour nursing care alongside doctor-recommended rehabilitation. The rehabilitation and nursing team assists individuals recovering from surgery, injury, or illness, with the goal of helping them return home safely.

Care services include physical, occupational, and speech therapy, with personalized rehabilitation plans designed to maximize recovery. The center also offers wound care supervised by a physician specialist and supported by certified wound care nurses, as well as physician-supervised, round-the-clock care for clinical needs.

The continuum of care extends to Palliative and Supportive Care Services, and licensed social service staff assist residents during transitional periods and throughout their stay. When residents are ready to return home, the team may arrange Home Health or other supportive programs to ensure a safe transition.

Daily life includes stimulating activities tailored to individual needs to help improve quality of life. A certified dietitian ensures meals are nutritionally balanced and meet medical and faith-based dietary requirements, supporting nutrition as part of the recovery process.
