About Farmerville Nursing And Rehabilitation Center, LLC

Farmerville Nursing and Rehabilitation Center is a skilled nursing and long-term care facility in Farmerville, Louisiana, with a licensed capacity of 117 residents.

Family experiences at the facility are sharply divided. Some families report attentive, compassionate nursing care—one described nurses as "very compassionate" and noted that staff provided honest, detailed communication about their loved one's condition. Another family praised the care provided to a resident with a tracheostomy and feeding tube. However, other families report serious lapses in basic care: one family's loved one did not receive scheduled medications during her stay and had to wait hours for assistance with toileting after pressing the call button repeatedly; another family brought their relative to an emergency room within days of admission due to a bacterial infection attributed to inadequate hygiene practices. These contrasting accounts suggest inconsistent care standards across shifts or patient populations.

The facility holds a 2.8 out of 5 Google rating based on 11 reviews, with families split between five-star and one-star assessments. Prospective families may want to visit in person and speak with staff members directly, as reviewers noted that personal observation and honest conversations with staff can help clarify what care looks like in practice.

It seems like an older place that got an outside makeover, but all in all its Nurses are Very Compassionate (I went on a Sunday). They answered my questions and were very honest about my patients abilities.

Girlfriend is a current resident at Farmerville Nursing Home. I picked her up on March 13,2026 and by March 15,2026 I had to bring her to the emergency room at Oschner Monroe. She stayed there several days due a bacteria infection from the staff letting her sit in a wet/spoiled diaper. If you value you loved ones do not send them there

Looking at past reviews I was scared if I wanted to send my mom here with 2 stars…..Let me tell you my mom had a trach and feeding tube and was getting little to no oxygen and I paid very close attention to her and her care and they did an Awesome job with my mom. I have zero complaints on this place and I’m glad I went threw with my decision to bring her here because not only did she get good care but I was able to be educated on things I knew nothing about.I would like to thank each and every last person that has dealt with mom (Mary James) on the trach hall I don’t remember everyone’s name but I can name a few like keyonna and theresa especially (Nashante) huge help and all the nurses and caregivers and also I would like to thank the nutritionist and last but not least I would like to thank the respiratory team and if I missed anyone thank you also because we couldn’t do it without yall and I’m very greatful for people that dedicate themselves to take care of others more than ever because I never knew how important yall jobs were until I got put in the position to witness myself. Even when at times my family wondered about my mom we was able to put a camera in her room to monitor her 24/7 if we wanted which also helped us cope with her being so far away.So thank yall so much and yall have been a huge help to My Family and most important to MY MOM❤️🫶🏾 Special thanks to the older lady that sees my mom in the morning I can’t remember her name light skin lady I believe she is respiratory also but every morning she would say “say good morning Mary” and talk to her as she would work and it’s just something about that lady that I can’t put my hand on but I know she is God sent because she showed so much compassion towards my mom and it didn’t go unseen and is well appreciated🥹

This rehab facility is very nice and some of the staff have been there a long time. I encourage you to tour and talk with Jarvis or Tina. They are care about the residents and the community.

My daughter had brain surgery and couldn’t move her left side. Couldn’t walk. She was there one day. Never got her medications that day. Pressed the call button all day and received help one time when a man went looking for help. She needed help to get on the potty by her bed. Pressed the call button til midnight. Finally had nurse call Ambulance because my daughter was in so much pain from no medication (and the blood clots in her legs) It was a terrible experience. 3 night nurses checked in then disappeared. It was terrible there. I tried taking away the one star but it’s to late. I’m praying for all those poor residents there.
