The Fairoaks rehab facility took care of my mother after leaving the hospital. They did such a good job! The nurses were great, the physical therapy team was great, the administrators were great, and the food prep team was great. Basically the whole experience was very good, and I recommend them.

My brother (55M) has lived with schizophrenia and bipolar disorder his entire adult life. He was recently diagnosed with stage 4 colon cancer and given 6 months to live. Following a hospital stay, my father was given 24 hours to select a facility and chose Fair Oaks Health & Rehabilitation. My brother was admitted on 5/30/26.<br><br>What we experienced in just 5 days: Our family was fully engaged from day one. My father visited my brother the evening he arrived on 6/1 and returned on 6/2, and 6/3 — talking to multiple staff members each visit and providing whatever information they requested. We were not a family that was absent or uninvolved.<br><br>On 6/3, the night supervisor approved my brother to leave with me for dinner — I signed him out, he signed out at the front desk, and we returned without incident.<br><br>On 6/4, my brother called my father to say he was being Discharged that day at 5 PM. My father — who held power of attorney (POA) and was the designated contact — had zero prior notice. When he called Fair Oaks, Mr. Felix Nwogve, the Administrator, confirmed the Discharge, vaguely citing something was unapproved and claiming my brother could care for himself. My father had personally delivered the POA to the facility on 6/1. Mr. Nwogve claimed it wasn't on file.<br><br>My father vehemently pushed back on the Discharge as he and I weren't in town for the day to supervise this Discharge and make sure my brother was covered.<br><br>What happened next is unconscionable. My brother was discharged anyway, an hour earlier than planned back to the hospital that recommended him for Fair Oaks Health & Rehabilitation. When my brother arrived at the hospital, the hospital didn't know he was coming so he was not recognized as a return patient. My brother refused treatment and disappeared before the hospital was able to figure out what was going on. My brother was missing for 30 hours before being found safe after midnight.<br><br>Despite our family's constant presence and involvement, this Discharge was carried out without honoring the POA and without any safety plan for a patient who lacked decisional capacity. He may appear independent in certain moments, but he has stage 4 cancer, suffers from serious mental illnesses, and is clinically unable to make sound decisions for himself. Discharging him without his authorized representative's presence and a plan to ensure his safety was not just negligent — it was a violation of his rights.<br><br>In addition, when the papers were finally given to us a week later, we found out this action wasn’t a Discharge, but a Transfer of my brother. Initial review of Discharge procedures versus Transfer procedures shows that Discharge procedures are much more regulated and involved which leads one to believe that something undue occurred due to the change in action.<br><br>Mr. Nwogve is responsible and his actions should raise the question of whether he should be working in healthcare. His disregard for patient safety and misrepresentation of facts are unacceptable.<br><br>Based on our experience, this is not a place we would ever recommend. The facility smelled terrible, consistent with the neglect we witnessed firsthand. Believe the 1-star reviews — they are true.

Our family member had a good care experience here. (May 19-June 3). While all the therapists did a very good job, Hannah in speech therapy was excellent! Likewise, nurse Meh was diligent in following up. Nesi, the social worker went above and beyond to ensure the discharge went smoothly.
