Our experience started off a little rocky, but once my loved one became acclimated, the facility and staff were truly able to shine. She was only there for two weeks for rehabilitation, but during that time she was treated with kindness, patience, and respect. One of the most meaningful things I witnessed was seeing her sit and read a book for the first time in several years—a small moment that spoke volumes about how comfortable and supported she felt. The nursing staff were exceptional. They not only provided excellent care for her, but they also supported me through the emotional challenges of being a caregiver. Their compassion, understanding, and willingness to listen meant more than they probably realize. Thank you to the entire team for helping make a difficult time a little easier and for treating both patients and families with such care.

Ambulatory care send me here for hospital stay my baths was in the mornings. the staffs are kind the OTC seems to have better cares then CNA and nurses I was taking Care of from them OTC instead. For my first appointment with a nurse was nice she left my jonnies on me i was warmed and comfortable she had a wheel chair ready for me to. As for the one that does therapy its really the top of the hemi walker that u suppose to use. All I got from the other staff was discharge notice and to do it your self ,and to where clothes instead of Johnnies, and my shower was suppose to be on Wednesdays this other nurse way was like. I told her I was taking care of this morning from a CNA and i am ready to go then she starts talking about where's my clothes i need clothes she came in with some clothes that she got from downstairs and put them on me in a rush. I was not comfortable at all. The same nurse, she took me to my follow up appointment with clothes on instead of johnnie's because I'm a long timer, so I had to inform her that I'm a short timer when I gotten back she didn't put my Johnnie back on which was a struggle because I was sick from my injury so I had to put them on myself, because she never returned to me. I was left in my wheel chair i had to elevate my leg also. and had to keep them informed of my next dosage because they said they can't remember me. Need a non dietary cart and in the food cooking For Short timers. need to keep short timer away from long timer. My bed was not comfortable jus to small I had to use my wheel chair to elevate my sick leg. my first with long timer very chatty talking and going in and out of the room letting me know the room is really for her. Then couldn't leave my curtain alone had keep pulling it back making her way across my way talking about I need some sun. Then ive gotten moved that same day to shorttimer stay with large size bed much better for my sick leg. Second patient long timer was quiet left my curtain alone her and her company stayed on her side of the room all the patient would say is hi and i said hi back to her rest time for me. a couple of days after they moved her elsewhere and brought third patient in with her company she was loud the CNA and nurse came in and started tampering with the curtain she was making room for herself to commune with the patient an her company. The last two days after therapy was done things was bazzaarsh. Other nurse and cna had that patient on my side of the room with her company jus chatting away and curtain fully pulled back across this time ever since that has happened I've caught something allergy, cold system like, they couldn't cure it i just kept coughing and sneezing. for the two last days of my stay My bed was changed to a different size again the bed wasn't made everthing was off i had to spread a comforter over the bed till she came. I had to keep getting up from my bed to pull my curtain across so they don't see me. I was uncomfortable its like my hospital stay was cut short no body came to see me nor bath me CNA was jus coming in preparing my bath and giving me my meals but never returned to bath me just kept coming in asking me if i needed some help or pull the red button when im fininsh bathing. So i told her aren't u suppose bath me. I really wish they should of told that patient company not to come by my way guess that was my notice. My last day was they jus handed me breakfast and gave me a large bag with just home care papers they took my wheel chair away and who ever was picking me up had to find one for me i was using an hemy walker till he found me one. I called them sometimes they would answer and other times they be like," I'm really aren't your cna or nurse". 4 need some kind of device for short timer to use like a computer or tablet.

NEVER take your loved one here. This place is dirty, it smells, and the level of care is unacceptable. The staff are inattentive and rude. When visitors are around, they suddenly act nice, but don’t be fooled. When you’re not there, your loved one is not being treated with basic dignity or care. My mother, who was there because she suffered a stroke, fell out of her wheelchair and HIT HER HEAD. How does that even happen in a facility that is supposed to be monitoring and caring for vulnerable patients? She was in severe pain, crying, and clearly needed help. The staff’s response? “If you want, she can go to the emergency room.” No urgency. No empathy. No accountability. Just a careless, dismissive attitude. That alone tells you everything you need to know about the level of care here. Not every staff member was like this, but enough that it creates a dangerous environment. This incident happened on the 3rd floor. I don’t care what kind of response this facility tries to give to justify this, there is no excuse. This is TERRIBLE care. If you care about your loved one, do not bring them here.

The worst place to ever bring your loved ones. The nurses/CNA's don't care. They let the patients sit in their own pee/poop all day, which led to my mom getting a UTI. You have to call them directly just to check up on them because their nurse call buttons "don't work". Bad attitudes and very rude. One nurse told me to assist my mom with the bedpan, although I am not a medical professional. She also fell and hit her head in their care and said "it's not their fault because they can't monitor all the residents at the same time", the social worker is very dismissive when you explain your concerns and how unsafe it is. They will also take weeks just to transfer them to another facility when you ask. They will also say you have to pay out of pocket to move them. The physical therapist gets frustrated easily when you can't perform a task to his liking. If you hate your mom/dad/whoever, then sure.. send them here. Just know they will be neglected, mistreated, and abused. DATE 2.5.26 (My response to their response): This response reflects the same lack of acknowledgment and reassurance that families can expect when raising serious concerns about their loved ones’ care. Despite my request for a formal investigation, I provided a clear and detailed timeline of each incident, supported by photos, videos, and the names of staff involved. Those concerns appear to have been dismissed rather than meaningfully addressed. This is the type of care you will receive from them. So thank you, Elmwood Nursing Home, for responding and showing how you address families who are concerned about their loved ones. No apology or empathy. SMH.

Terrible from the moment you walk in. "Receptionist" was unwelcoming and unhelpful as to where to go. My husband, a stroke patient, fell on the second day there. Took him home following that incident. Next day got a call from the billing person said I needed to sign form so they could bill the overnight stay. Awful.
