After a hospital stay I had a family member come to diversicare for rehab. The therapy team did a great job. They even had a practice kitchen and stairs in the gym to work on. They worked hard and got him back on his feet and he was able to return home. The team did a great job with communication from the initial team meeting with the family to ongoing communication. It was a couple hiccups along the way but the DON and Administrator and the team was quick to address any concerns. I felt like I could reach out and talk to anyone and they didn’t mind helping. Dr Bolen the medical director had a great bedside manner and I also liked that they had a nurse practitioner in the building full time also to address any concerns or emergencies that might arise it definitely helped to ease our concerns. Also I would just like to say that I also noted how clean the facility was and never had that institutional smell that some places can have. Also it seems like the staff took care in making sure the rooms and common areas looked nice and homey. I would definitely recommend this facility to anyone looking for a place for there loved ones for short term rehab or longer. Also I would just like to tell people reviews are a good starting point but if you are looking into a place nothing beats just going and seeing for yourself. I am glad we did and decided on this place.

Thank You to Brenda and the PT Team I want to give a special thank you to Brenda, one of the best nurses at this facility. Despite some difficult experiences with a few staff members on first shift, Brenda stood out in the best way possible.She was sincere, caring, patient, and showed genuine compassion. You can truly tell she cares about her patients and takes pride in her work. Her kindness, daily check-ins, and support made my temporary stay much easier.I also want to give a big thank you to the Physical Therapy team. They were professional, encouraging, and did an absolutely great job helping me through my rehabilitation.Overall, this facility has some good people and strong rehabilitation services. However, my biggest concern was the lack of friendliness and helpfulness from some first-shift nurses. Some of the comments made toward my family were hurtful and unprofessional, which affected my overall experience.Because of that, I personally would not return, but I am truly grateful for Brenda and the PT team. They made a major difference, and I cannot thank them enough for their care, compassion, and dedication

Diversicare in Montgomery put my mother at serious. Action will be taken! We had to call 911 ourselves to get her back to the hospital, where she was admitted for heart failure symptoms. The only nurse on duty (in the building) said nothing was wrong She showed clear signs of congestive heart failure (fluid in lungs, severe swelling, shortness of breath, extreme weakness).. There was no physician present. Staff then tried to make me sign a blank AMA discharge form with no doctor signature or witnesses. I refused. This was a complete failure of care, judgment, and basic patient safety. Families should be extremely cautious.

The entire team at Diversicare made the experience with our mother a manageable one. The staff at the front desk, nurses, and nutritionist were all very kind and communicated with our family to help us understand the overall care plan. We appreciate their professionalism and attentiveness.

This is NOT the place to bring loved ones who need care! This place has an impressive commitment to doing the bare minimum! Your loved ones will be left unattended, mutilated and left to deteriorate! Muscle atrophy, loss of strength and mobility are all guaranteed. Meanwhile, range of motion exercises and any kind of road to recovery is out the window! Your person needs help eating, bathing, using the restroom? GOOD LUCK getting that! If your expectations are low, they’ll meet them beautifully.
