Everyone is so nice and helpful. Nursing, therapy, activities, housekeeping and dining. The food is good. I feel very comfortable here.<br>The therapists are very competent.<br>I have had a wonderful stay and highly recommend Wellspring.<br>The facility is well maintained and clean.<br>I am glad I chose this facility for my rehab.<br>Cindy

I am absolutely disgusted with this administration of this so called rehabilitation facility. First of all don't take on patients when you're understaffed because you're putting people's lives in jeopardy. My 80 year old father who has medical issues including dementia fell and broke his hip. After spending 8 days in the hospital they recommended Covenant Skilled Nursing and Rehabilitation Center so he could return to either his independent living or assisted living. I don't think those who recommended this place knew he would be placed in this facility where therapy was only going to be a half an hour a day if he felt like it or left in bed for almost the whole day. Apparently they need lessons on dealing with dementia patients. I had to constantly remind them that they need to put his food tray in front of him as there were many times they dropped it across the room and he couldn't get out of bed to get it. Unfortunately due to his dementia, he forgot that he had to push the red button for help and they said they would make a note to complete regular checks; however, that didn't happen and each nurse or cna would say the same thing, another note. There were several times that when I came in the red button was placed out of his reach. Then when he pushed it because I told him to do so, the average amount of time it took for a nurse or cna to respond was more than 30 minutes because they were stationed to the other unit and had to walk across to the other side of the building. I was there every freaking day and sometimes twice a day for 19 days and not once did I get any updates on his care from their doctor, the charge nurse, or social worker. Keep in mind, those who have love ones there, they give bath or showers at night and if the person says no or don't want to wake up they won't get cleaned. I took my dad to get his staples out and the doctor had to write in the orders that he can be shower so he could get one because she noted 3 weeks after the surgery he still had the markings and solution around the bandage. Again every person I spoke with they would say they're understaffed and so many critical patients that they can't keep up. Several nurses asked me in the afternoon, early evening isn't he going to therapy today? My comment should have been, you should be telling me he is. What the heck 😳 I finally could not handle after 3 days in a row, i arrived afternoon for various reasons and he was not awake for the day, not even dressed, hasn't eaten, have to wait for a long time to use the bathroom after pushing the red button. I demanded my dad to be discharged, so i can get him the real help he deserves. Rehabilitation is not what this place does and if you got someone that did help you you truly are lucky. Shantell, Kim, DeeDee, Jill, and Aurora, I want to thank you for trying your best. It's ridiculous that you have to care for so many patients in a shift/unit. Karrie, I also wanted to say thank to you for being the only person in the office that did what you said you were going to and helping me after his visit to the doctors.

My elderly father was recently sent to this facility after breaking his back. The rooms and interior of the building are very nice, clean and bright with the rooms with a hotel room like feel. Although I never ate their food my father said it was pretty good and had no complaints. The low review score comes in at the the nursing care they provide. It’s quite awful. It would on average take someone 15-20 minutes to answer his call button. After being there for a week I had to ask them to help him to shower since they had not done any self care on him. Whenever I had questions the nurses I spoke with seemed clueless. With one actually that kept saying, “I don’t know this is my first time with your dad” until I suggested she go look at his chart and comeback. Also his toilet was broken leaving poopy water one the floor and my father told them 3-4 times about it but they never fixed it or cleaned up the mess. I would NOT recommend this facility if you are relying on any one on one care. Because I noticed they were more interested in standing around chit chatting with each other them actually helping their patients that they are being paid for to do. .

This is a very nice place with just a few flaws.The biggest problem they have is the cafeteria and the meals they serve,they would come tell my wife what they had on the menu,she would pick what she wanted and then it wouldn't be what she ordered,why even say they have a menu when they just give you what they want to give you,and 97 percent of the time when you got your meal it was so cold you didn't want it,my wife got a waffle that was so hard it curled right up,she couldn't eat it of course.I had to go out a lot and order something from the local restaurants,the only thing that they had good was the fish,but they only had that twice in the 3 weeks she was there,she would get mashed potatoes and it said gravy too,but the gravy wasn't there, dessert and no dessert was there,so they either need to get a good cook who can handle the meals or hire more staff for the cafeteria,there has got to be more organization in the cafeteria.Now I would like to mention the good side of Wellsprings,the. Nursing staff is excellent,Candace and Aushan was A plus in my book,and other nurses and the aids like Jennifer,dimekiah,Cletoria, Ericka, Cecila Catherine,Nika,Timeko,Demetria, and Olivia they really liked their jobs,and you could tell that by their very caring attitude.I give wellsprings 4 stars because they need to improve on their cafeteria.Otherwise I would have given them 5 stars.

Very Professional, very friendly, extremely clean. Medical personnel very concerning about their patients. Food very tasty, but sometimes cold.
