Conejo Valley Home Care

Conejo Valley Home Care is a family-owned Residential Assisted Living community in Thousand Oaks serving people ages 60 and older who are no longer able to live alone and independently but who do not require skilled nursing care. The family behind the community has been active in the elder care industry for over 20 years and applies that experience to daily operations.

Care includes hands-on daily assistance with activities such as bathing, dressing, and personal hygiene. Residents are provided three home-cooked meals and three nutritious snacks each day, with accommodations available for special dietary needs.

The community is led on a day-to-day basis by Melissa Shubin, a Registered Nurse since 2010 who specializes in geriatrics. Her background includes intensive care, case management, serving as Director of Nursing, home health, and palliative and hospice care. She is closely involved in daily operations and focuses on incorporating socialization, nutrition, and therapy into each resident's care plan.

Amenities include multiple home entertainment areas for recreational activities and family visits, a spacious living and dining room, a secure fenced yard, and patio and lawn seating. Safety features include a personal body call system and floor mat alarms for fall prevention. Transportation and escort to medical appointments is available for a service fee.

Programming includes weekly live music from a professional musician with sing-along requests, along with a Recreational Specialist who provides social activities, bonding exercises, and assistance with range-of-motion exercises.
