I visited this rehab facility with some friends and had a very positive experience. The staff was professional, caring, and attentive, and the environment was clean, calm, and welcoming. I was impressed by the compassion shown to the residents and the overall atmosphere of support and dignity. Very well organized and encouraging place. Susan D

This place needs to be shut down. I could say 100 negative things about this god awful facility. I had put out a complaint with 311 because of how my father was not being cared for. He fell out the bed 4 times while being a fall risk, they didn’t communicate about him not taking his medication, I had to make light of when my father did not look well and demand for him to see a doctor. I walked in one day to see my father hanging off the bed in his sleep while a nurse was in front the door about administer medication. You would think she would have called for assistance to prevent a fall. They were also giving him melatonin to calm him so they wouldn’t have to deal with them. I didn’t know that until I asked why does my father always look out of it and not fully alert. There’s too many negative experiences for me to share here but I warn you, do NOT settle for this place. My father was found unresponsive and passed away at this facility on March 28th 2026 only 3 months after being admitted for short term rehabilitation. If the lack of care and attention wasn’t enough, they handed me a bag of soiled nasty stinking clothing with roaches, gnats, and flies coming out the bag when I went to retrieve his belongings…PROOF that they weren’t even washing his clothes let alone valued his health and well being. How do you hand a grieving family a bag of urine spoiled clothing???? No empathy. No care. Just a staff that gets off watching your family members suffer. This place is the walking dead. DO NOT SEND YOU LOVED ONES HERE. I have to bear the loss of my daddy while these soul less monsters go home and kiss their children. The atmosphere is miserable, I feel sorry for the residents, and if you decide to let your loved ones rehab here be prepared to be with them every minute of the day to guarantee they walk out alive. The oath they take to do no harm is non existent. I hope this place shuts down. I have nothing good to say about this place.

I love Concord nursing home. My mother has been in here for months too as well taken care of more than she was at home. They really take the time out to talk to you and see if you need anything and they are dedicated staff. I would recommend this place to anyone.

I very recently went to visit my grandfather, and I was surprised to see the deplorable state this facility is in. According to the website and testimonials it seemed so nice and clean. The staff so warm and hospitable. But when I went to visit the facility over the weekend, it was dirty, it smelled moldy, the atmosphere was very depressing, the staff unprofessional, the patients looked uncared for, the staff lacked empathy. I’m very worried about my grandfathers state of rehabilitation in this place. It’s sad to imagine the type of care they are receiving when we aren’t around, the environment is not giving hospitality. I hope someone does something about the state of this place soon. Because if nothing changes, we’re going to have to find him a more suitable rehabilitation facility for him to heal in.
