About Columbia Healthcare Center

Columbia Healthcare Center is located in Evansville, Indiana, and offers a full continuum of senior care services, including short-term rehabilitation, memory care, long-term care, respite care, hospice care, and outpatient therapy. The community emphasizes a person-centered approach, aiming to become an extension of each resident's family while helping them and their loved ones find answers, solutions, and peace of mind.

Short-term rehabilitation is designed for those recovering from strokes, cardiovascular difficulties, orthopedic surgery, and other debilitating conditions. The program includes physical, occupational, and speech/language therapies delivered by an interdisciplinary team of doctors, nurses, therapists, social workers, and other care providers. Through the Road to Recovery program, exclusive to American Senior Communities, the care team, patient, and family meet within the first 72 hours of admission to map out discharge goals. Additional rehabilitation services include wound care, fall prevention, contracture management, wheelchair positioning, mobility training, and strength-building and aerobic conditioning programs.

The community's memory care program is structured and research-based, using a person-centered, wellness-based model that incorporates stimulating daily activities to support independence and a healthy spirit. The program focuses on maintaining a secure environment where residents experience a sense of belonging and meaning each day, including a Meaningful Day activity program built around four domains of wellness.

Long-term care is available for residents who need assistance with daily tasks due to illness, injury, or frailty, and can no longer be cared for at home. Care is provided under the guidance of an attending physician by licensed nurses, specially trained health care aides, and an interdisciplinary team, with services including social activities and physical, occupational, and speech therapies. The settings are described as home-like, cheerful, and secure. The community accepts private pay along with a variety of payment options, including Medicaid for qualified applicants, with Medicaid specialists available to assist with the application process.

Respite care is offered for short-term stays, giving family caregivers a break to travel, start a new job, recover from an illness, or pursue other interests, with the option to extend care if needed. Hospice and palliative care services are provided in partnership with local hospice companies, offering skilled nursing care, spiritual and bereavement counseling, medical equipment and supplies, social services, and pharmacy services through an interdisciplinary team of physicians, nurses, aides, volunteers, social workers, and clergy.

I was needing to walk after back surgery three years ago. The therapist worked hard with me, the CNA cared for me ( sometimes they were short staffed like in the hospital, one person working two halls) due to schedules & cancellation. The nurses checked on me every day & night to give meds. The staff was friendly and helpful. I was bedridden when I came but was able to walk when I went back home. Thanks to Columbia health care!
