TERRIBLE! If you even slightly like your family member, do NOT send them there. Low staff. Have to beg for a shower more than once a week. ZERO ACTUAL physical therapy.

Carroll Healthcare Center Inc is an assisted living and skilled nursing facility offering memory care, rehabilitation, long-term post-acute care, and ventilator care in Carrollton, Ohio. Located in a small, rural town, the facility provides a continuum of care with individualized attention from a compassionate staff, many of whom have worked at Carroll for 20 or more years.
The facility operates in a home-like environment rather than as a traditional nursing home, serving residents from short-term stays to 24-hour care. Carroll offers ancillary services including eye, dental, audiology, and podiatry services, as well as transportation to health appointments outside the facility. The medical team specializes in caring for clinically complex, high-acuity patients, including advanced wound care with board-certified consulting vascular surgeons and nurses trained in wound care, as well as specialized respiratory support through a dedicated Vent Unit staffed with respiratory therapists and nurses with extensive ventilator experience.
The facility features Adele Court, an Alzheimer's and Dementia Enhanced Living Environment with lounges, multi-sensory rooms, music rooms, and shower rooms designed as peaceful, home-like spaces. Carroll offers expanded activity programs including music therapy, a multi-sensory room with optical illusions and sensory stimulation, an award-winning intergenerational art-making program for residents with dementia and Alzheimer's disease, and palliative care that combines skilled touch with compassionate presence for those in later stages of life.
Residents have access to a range of programs and activities selected based on their interests, life skills, and participation tolerance. The facility's therapy department works with short-term, long-term, and subacute residents with the goal of returning them to their prior level of function. Care for residents on ventilators includes noninvasive ventilation support such as CPAP and BiPAP, as well as care for those with tracheostomies, all managed by a skilled team with individualized weaning programs designed to support eligible residents.

My grandmother has been a resident here for about 6 months and we have had nothing but good experiences. The staff is very dedicated and friendly when it comes to both visitors and residents. They not only make sure the medical needs of their residents are met but also meet the personal needs and build relationships with their patients. Any concerns that we’ve had have been addressed immediately and they are always doing everything they can to make the residents comfortable. I couldn’t ask for a better place for my grandmother to be and I am immensely grateful for the team here.

Incredible facility! My uncle was admitted to this facility a month ago and received excellent care. I was there morning and evening and spent quite a bit of time watching the kindness and patience every staff showed to him. I also listened to the interaction of staff with other residents when they did not know I was across the hall. One resident needed to be fed. The staff told her to take her time to eat. Then, as the lady struggled, the staff said, "I am here to help, and I am happy to be here with you and to give you all the time you need." This is a small example of the kindness that was demonstrated again and again with each staff. My uncle also required a puree diet, and the kitchen worked very hard to find food he would eat. They even made him peanut butter and jelly milkshakes. The therapists communicated with me frequently and made time to meet with me. Cheyney was diligent in contacting me to find a time for me to meet with the speech therapist. Nicole, the physical therapist, took time to review her goals and answer my questions. The aids at this facility restored my faith in young people as they were willing to care, really care and showed no frustration or disgust for all the various tasks required to care for elderly people who are not able to care for themselves. I want to give a shout-out to Jessica, Jaden, April, Loren, and Harmony. Also, the nurses were stellar! Andy met us when we arrived close to 9pm. She already had the med chart ready. Found a good meal and took time to fulfill my many requests. I found all the other nurses patient and thorough. Additionally, Heather helped me with all the paperwork and navigating the logistics of insurance and legal matters. Lastly, Sarah Davis went above and beyond on all aspects. She arranged a meeting for me with all the key players for my uncle's care. She promptly answered my millions of questions and responded quickly to my requests. Carroll Senior Care Center is a gift to the small community of Carrollton and the surrounding area! If you are looking for a place that provides excellent care, look no further!

My Dad is here for rehabilitation and has received excellent care along with people who care about doing a good job. I have observed this from the cleanliness of the facility, the staff in dining room, nursing staff, and the physical therapy staff. I have visited different hours of the day and see the same. Special thanks to Rachel at front desk, Channey the physical therapist, Sarah -IPCO /SSD and nurses and aides for Dad's room 115. Everyone I talk to about my Dad is helpful and friendly. If my Dad needs to stay we hope he can stay here!
