My father was transferred here from hospital after a fractured spine. He was there for several weeks for rehab. PT and OT were with mixed reviews. Some were phenomenal where times my father would tell me therapist would leave after 10 min and he would Demand the full 30 min. Chief complaints were mostly about the food and long response time from using call bell. Shout outs to nurse Steve for excellent communication and follow up, the CNA's that showered and provided excellent morning care for toileting and dressing for the day ( sorry their names escaped him) and Angel, his social worker for the great communication in helping to coordinate his discharge and everything he needed once home for continued therapy. Angel kept me informed (I do not live local) and she stayed in touch with me, continued assisting after discharge when there were some issues with equipment delivery. I appreciate her willingness to go the extra mile to make my father's transition home seamless. Thank you, Angel!

I have been a resident of this facility since 1/14/2026 because of having my knee replaced. The entire staff here has been excellent. The Administration has been great with communication between my doctors and the hospital. My nursing staff has been amazing. They are very caring, attentive and have a loving nature. I have been here twice before 2022 and was very displeased with the level of care and cleanliness of the facility. This place has done a complete 180 degree turn within every department. Food service does need improvement with maintaining the temperature of the meals. My food every meal has been cold to barely warm. Always cooked correctly and looks great in presentation but failed to maintain the temperature. The staff that was assigned to me was awesome. From housekeeping to the CNA's to the nurses, they always treated me like a King. I am so pleased with this place and recommend it 100% to anyone in need of a rehab facility.

This is the worst place to send a loved one. I have been here for 5 days (since Friday) and it's been nothing but chaos since I arrived. I came for iv antibiotics and spent the first 2 days waiting on them to come from the pharmacy along with my pain meds. I was told they only order meds from Tampa or Orlando. So they finally got them on Sunday. Now here it is on Tuesday, 2 days later and they are out of my meds again. How, why? It makes no sense. The CNA's don't answer the call lights, sat here for over 40 minutes multiple times with my call light on (good thing I wasn't dying). The nurses are so understaffed it takes over an hour to get any help from them. I had to call the operator and ask for my nurse because nobody would answer my call light. The nurse last night was new (her first day ever in this field) and they threw all these patients on her with no help or guidance. She had no idea how to hang iv meds, she cant draw blood from a port, she was so nervous she was shaking trying to flush my iv. The food here is terrible. I lost over 10 lbs in 5 days. They feed you enough for my 5 year old. The cleaning people left wet towels on the floor for days that the nurses used to clean up iv fluids that drained onto the floor. I walked in the bathroom and saw 2 roaches on the floor that ran into the wall when I walked in. The toilet in the bathroom leaks.

This facility had been moving in the right direction, and then they started slipping in major ways. Broken furniture that isn't fixed or replaced unless complained about repeatedly, for example. The biggest problem is missing personal items. My father's glasses, which is pretty major. Missing articles of clothing even though every single item is labelled with his name, he has enough clothes that I should not walk in on any given day and find him wearing none because they are stolen or strewn around the hospital. Inconsistent hygiene maintenance for the patients, most of whom are unable to take care of themselves. Also, a permanent on-site nurse practitioner that was to administer a particular treatment for a condition and then never did it, and didn't communicate it to anyone else. Communication between management and staff is poor and even when pressed, simple but mandatory advisories are overlooked or simply ignored. Family should be notified every single time their relative falls no matter what, and medication changes should be advised BEFORE implemented, not at the end of a 14-day prescription. These are serious violations that could have legal ramifications. It's a real shame because the CNAs and nurses truly care for the patients and they show it every day, but they are very understaffed and they cant be as effective as they want to be. The only reason I gave two stars is because of the CNAs and nurses because management and the owners haven't earned more than one.

I came to Brentwood from the hospital for rehab and to gain strength in my arms and legs. The therapy team exceeded in helping me meet my goals while at the facility. My nurse Steve was awesome, I enjoyed Cassandra in the activities dept. My Social Worker was very kind, and respectful explaining my discharge home. My CNA Karen was the best CNA I had at facility. I truly had a great experience. Thank you to all of the staff that try to do a great job! Thank you Mr. Gary Whittle
