My father in law was here a short time, and I have to say, everything was excellent. A few staff members deserve specific shout outs: Nurse Dan is just so kind and attentive, I never worried about care when Dan was around. Kaydi, the CNA might be the most cheerful person I’ve ever seen, always smiling. The unit clerk, Jay, is so on point with scheduling appointments and keeping us up to date. Dawn, the director of hospitality, has an incredible personality and made us feel welcome from the second we walked in. All in all, just a great experience

The Staff here are extremely knowledgeable, very positive and helpful. They listen to me when trying to help them understand the needs of our resident while he was here. Great activities for Everyone. Beds were made every day rooms were clean had no issues. Food was wonderful and you can choose from ultimate menu. If there was something you didn’t like. I would recommend autumn Lake/Southgate for anyone needing rehab or long term care.

I am so happy with the care my Uncle received. Dan his nurse was always very helpful and informative to us family. Dawn was such a huge help with understanding everything and the sweetest for always trying to raise my Uncles spirits. Thank you Southgate for job well done!

I am extremely happy with all the changes the facility has made. Between the amazing nurses like Kia and Michelle, the great therapy staff like Lisa and Sheri, and Dawn with Hospitality is so helpful! The facility is so much cleaner and other staff getting friendlier! Happy with Improvements!
